  The establishment does not have an employee health policy regarding the prevention of foodborne illness. We provided the establishment with a copy of the DC Food Code employee medical referral, reporting agreement, and interview forms.
• Proper use of restriction and exclusion
  The establishment did not provide proof of an employee health policy for the prevention of foodborne illness.
• Adequate handwashing sinks properly supplied and accessible
  There is no handwashing sink installed in the establishment.
• Insects, rodents, & animals not present
  There are openings in the wall adjacent to the 3-compartment sink, ceiling/wall junctures in the rear of the establishment, and the toilet room. The toilet room ceiling has holes and the lower parts of the walls are open. There are additional openings in the ceiling in the front of the establishment.
• Food and nonfood-contact surfaces cleanable, properly designed, constructed, & used
  The shelves in the rear of the establishment are constructed of particle board and they are not nonabsorbent.
• Warewashing facilities: installed, maintained, & used; test strips
  There is only one drainboard on the 3-compartment sink.
• Plumbing installed; proper backflow devices
  There is no air gap beneath the spray hose at the 3-compartment sink.

  The mop sink is not constructed of approved materials for commercial use. The establishment has a plastic laundry tub installed.

  The garbage disposal unit (food waste grinder) is directly connected to the drain in the bottom of one of the 3-compartment sink's compartments.

  The toilet room door is not self-closing.

  The toilet room handwashing sink is not secured to the wall. There is a gap behind the sink and the upper portion is not secured to the wall along its width.
