    Observation: A large dented food can was found in the dry goods storage area.
    Correction: Discard any damaged boxes, cans, and food that potentially are contaminated or adulterated. Correct By: 07-Apr-2015
  • Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER (corrected on site)
    Observation: Raw hamburger was observed stored over raw fish in the 4 drawer cooler by the grill.
    Correction: Store raw animal foods in order of decending hazard to prevent cross contamination. Fish was moved to the drawer above and the hamburger moved below during inspection.
  • Critical: PHF/TCS, COLD HOLDING (corrected on site)
    Observation: Various food items were observed cold held at 44-45°F in the 2 door reach in cooler near the grill and in the waitstaff salad bar area.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. Coolers were turned down and foods were found to be within range later during inspection.
  • Critical: DATE MARKING - DISPOSITION (corrected on site)
    Observation: Cooked rice and bean mix in walk-in cooler has exceeded its date mark of 3/28.
    Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date. Mix was discarded during inspection.
  • Critical: SEWAGE - BACKFLOW PREVENTION
    Observation: A connection exists between the sewage system and ice bin drains in the waitstaff area and bars. Currently air breaks are present.
    Correction: Design the system in a manner that would preclude a connection between the sewage system and drains. Remove the connection with a proper air gap. Correct By: 11-May-2015
  • Critical: TOXIC SUBSTANCES - IDENTIFYING INFORMATION, PROMINENCE - ORIGINAL CONTAINER (corrected on site)
    Observation: Blue grill degreaser was observed in a bottle labeled Keystone orange multi-purpose cleaner.
    Correction: Provide containers labeled correctly for poisonous or toxic materials used in or by the food establishment. The bottle was discarded during the inspection.
  • Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED (corrected on site)
    Observation: Keystone orange multi-purpose cleaner was observed being used as a sanitizer in wiping cloth buckets.
    Correction: Change procedures and provide training to employees on proper sanitizing and use of toxic chemical following manufacturer's directions for use. Buckets were emptied during inspeciton. A new sanitizer bucket at concentration of 50ppm was made during inspection.
  • PROPER COOLING METHODS - CRITERIA
    Observation: Improper cooling methods are observed for chicken noodle soup in walk-in cooler. Observed soup in a large capacity container at temperature of 145.9 at beginning part of inspection (15min of cooling).
    Correction: Adjust methods or procedures to facilitate cooling as described in 3-501.15. Adjust method or procedures to cool foods from 135°F to 70°F in the first 2-hours and from 70°F to 41°F in an additional 4-hours or if prepared or received at ambient temperatures cool to 41°F in 4-hours. Soup was moved to an ice bath during inspection. Observed temperature at 100.2. Soup was moved to smaller containers to again increase cooling process. Correct By: 07-Apr-2015

    Observation: The consumer advisory is provided on the menu but is missing an asterisk or symbol next to it and each item that can be made to order.
    Correction: Provide an asterisk or symbol by the consumer advisory and each corresponding animal food served in a raw or undercooked condition. Correct By: 11-May-2015
  • LABELING - FOOD STORAGE CONTAINERS LABELED WITH COMMON NAME
    Observation: Flour or sugar was observed stored in an unlabeled container in the dry goods storage area.
    Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient. Correct By: 07-Apr-2015
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
    Observation: Meat products were observed stored on the floor in the walk-in cooler. Fish and fries were observed stored on the floor in the walk-in freezer.
    Correction: Store all food items 6 inches above the floor. Correct By: 11-May-2015
  • THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
    Observation: Three coolers were not equipped with an integral or permanently affixed temperature measuring device (2dr salad prep, 2dr grill, 2dr Beverage Air).
    Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device. Correct By: 11-May-2015
  • NON-FOOD CONTACT SURFACES - CLEAN
    Observation: Non-food contact surfaces of blender and can opener were observed soiled. Vent above dry good food storage has visible dust accumulations.
    Correction: Clean and remove dust, dirt, food residue, and other debris from all food and non-food contact surfaces of equipment. Correct By: 07-Apr-2015
  • DRYING MOPS
    Observation: Mops observed stored on the floor in chemical room and bottom of the mop sink.
    Correction: Store mops to allow them to air dry without soiling walls, equipment and supplies. Correct By: 11-May-2015
