Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER (corrected on site) Observation: Raw hamburger was observed stored over raw fish in the 4 drawer cooler by the grill.
Correction: Store raw animal foods in order of decending hazard to prevent cross contamination. Fish was moved to the drawer above and the hamburger moved below during inspection.
Critical: PHF/TCS, COLD HOLDING (corrected on site) Observation: Various food items were observed cold held at 44-45°F in the 2 door reach in cooler near the grill and in the waitstaff salad bar area.
Correction: Maintain cold potentially hazardous foods at or below 41°F. Coolers were turned down and foods were found to be within range later during inspection.
Critical: DATE MARKING - DISPOSITION (corrected on site) Observation: Cooked rice and bean mix in walk-in cooler has exceeded its date mark of 3/28.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date. Mix was discarded during inspection.
Critical: SEWAGE - BACKFLOW PREVENTION Observation: A connection exists between the sewage system and ice bin drains in the waitstaff area and bars. Currently air breaks are present.
Correction: Design the system in a manner that would preclude a connection between the sewage system and drains. Remove the connection with a proper air gap. Correct By: 11-May-2015
Critical: TOXIC SUBSTANCES - IDENTIFYING INFORMATION, PROMINENCE - ORIGINAL CONTAINER (corrected on site) Observation: Blue grill degreaser was observed in a bottle labeled Keystone orange multi-purpose cleaner.
Correction: Provide containers labeled correctly for poisonous or toxic materials used in or by the food establishment. The bottle was discarded during the inspection.
Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED (corrected on site) Observation: Keystone orange multi-purpose cleaner was observed being used as a sanitizer in wiping cloth buckets.
Correction: Change procedures and provide training to employees on proper sanitizing and use of toxic chemical following manufacturer's directions for use. Buckets were emptied during inspeciton. A new sanitizer bucket at concentration of 50ppm was made during inspection.
PROPER COOLING METHODS - CRITERIA Observation: Improper cooling methods are observed for chicken noodle soup in walk-in cooler. Observed soup in a large capacity container at temperature of 145.9 at beginning part of inspection (15min of cooling).
Correction: Adjust methods or procedures to facilitate cooling as described in 3-501.15. Adjust method or procedures to cool foods from 135°F to 70°F in the first 2-hours and from 70°F to 41°F in an additional 4-hours or if prepared or received at ambient temperatures cool to 41°F in 4-hours. Soup was moved to an ice bath during inspection. Observed temperature at 100.2. Soup was moved to smaller containers to again increase cooling process. Correct By: 07-Apr-2015
