Observation: Flooring in walk in cooler is beginning peel away and is also visibly rusted.
Correction: Repair equipment to good condition or remove from premise.

Observation: There is a consumer advisory stated on the menu but the food items that pertain to the advisory are not denoted.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
- KITCHENWARE AND TABLEWARE - HANDLED TO PREVENT CONTAMINATION
Observation: It was observed that spoons, forks, knives being stored above the 2 Door Prep Cooler where the food or lip contact surface could be contaminated.
Correction: Store utensils in a manner to avoid contamination to the food and lip contact surface.
- WATER FROM APPROVED SOURCE - SAMPLE REPORT
Observation: A recent water sample report for the establishment was not available, the water test result was looked up on the DNR website by inspector and was tested safe on 5/31/13.
Correction: Retain a recent water sample report onsite or online for review by inspector.
- PLUMBING - SERVICE SINK
Observation: There is no service sink provided in food establishment, dirty mop water is being dumped down the toilet.
Correction: Provide at least 1 service sink or curbed cleaning facility. Discussed options for a mop sink, and determined that a drained sink may only be required because they have effective ways to fill the mop water bucket.
- TOILET FACILITIES - RECEPTACLES - COVERED
Observation: Female bathroom is not provided with a covered waste receptacle.
Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.
