Observation: Employee observed assembling burger bun/toppings and toast with bare hands. Discussed with Owner proper glove use and when to use them.
Correction: Employees shall use single-use gloves, utensils, bakery papers or other approved means when handling ready-to-eat foods.
- Critical: PHF/TCS, COLD HOLDING (corrected on site)
Observation: Diced Ham and Par-Cooked Mushrooms were sitting out on counter top at the time of inspection at 47°F. Food employee moved items back into cold holding during inspection. Discussed with employee's the need to keep foods under proper cold holding when not in-use.
Correction: Maintain cold potentially hazardous foods at or below 41°F
- Critical: FOOD CONTACT SURFACES - WAREWASHING EQUIPMENT, CHEMICAL, TEMPERATURE, Ph, CONCENTRATION AND HARDNESS (corrected on site) (repeated violation)
Observation: Wiping cloth solution used to clean/sanitize food contact surfaces in the kitchen had no detection of chlorine bleach when tested. Facility has other sanitizer options to use in addition to bleach water wiping cloth solution. Solution was remade during inspection to proper chlorine concentration.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.

- Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site)
Observation: Chicken soup, mashed potatoes, and cooked chicken breasts in Walk in Cooler was not date marked. Soup was labeled, potatoes and chicken breasts were discarded.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

Observation: Wiping cloth solution had no detection of bleach at the start of inspection. Employee discarded solution and made fresh and relabeled the bottle. Minimum of 100ppm of chlorine bleach required in wiping cloth solution. Solution was remade to 200ppm chlorine bleach.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.

Observation: Chlorine test kit is not available for checking sanitizer concentrations of bleach water wiping cloth solution (in spray bottle).
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
- LIGHT BULBS - PROTECTIVE SHIELDING (repeated violation)
Observation: Lights located in kitchen (right when you enter kitchen) are not shatter resistant. The plastic covering is missing.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.

Observation: Current permit is not posted in establishment. Current permit will be mailed to Owner to be posted.

Observation: Faucet on mop sink has a watts 9D however the Y connector on the faucet makes it insufficient..
Correction: Provide an air gap on water supply side to protect water supply approved with the Y connection or remove Y connector..

Observation: A direct connection exists between the sewage system and a drain from ice bin in front.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Remove the direct connection.
