Observation: Employees are unaware of the illness reporting requirements to the person in charge. Employee health forms need to be completed and on file for review by the inspector.
Correction: Provide training to all employees on the signs and symptoms of foodborne illness and explain the requiremnts for reporting to the person in charge.

Observation: Pickled eggs were found in the cooler that were prepared in a private home.
Correction: Discard food from unapproved sources.

Observation: Several food items were found in the cooler that were not datemarked. Cooked hot dogs, ham, pulled pork, sloppy joes. All foods were ordered discarded. Repeat violation.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

Correction: Rearrange area so that toxic materials are stored below or away from food, equipment and single service items.

Observation: The Person in Charge (PIC) is unable to demonstrate knowledge of washing and sanitizing dishes and equipment. Owner did not know how to wash and sanitize dishes, proper datemarking or approved source of food.
Correction: The person in charge shall obtain training or training materials in the areas of [items] so they are able to demonstrate knowledge and train employees on proper food safety practices.

Observation: The interior of the ice machine is visibly soiled.

Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.

Observation: The owner's dog was observed in the bar.
Correction: Remove the animal from the premises and keep all prohibited animals out of the premises.

Observation: Residential pressure cooker and rotisserie are not ANSI certified or approved by the department.

Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
