    Observation: Utensils and equipment are not being sanitized after cleaning. Observed dishwashing unit to test at 0 ppm for sanitizer at time of inspection.
    Correction: Clean and sanitize all utensils and equipment before contact with food. Service was called for the unit. Until unit is fixed and operating correctly to sanitize dishes, the sink basins will be used. Correct By: 17-Jul-2015
  • Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED (corrected on site)
    Observation: The Multi-Quat sanitizer for food services was observed to not be used according to manufacturer’s use directions. Test of working container was at 0 ppm.
    Correction: Change procedures and provide training to employees on proper use of toxic chemical following manufacturer's directions for use. Bottle was emptied and new solution was added during inspection. Retest of bottle was at 200 ppm.
  • WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES
    Observation: A chlorine test kit is not available for checking sanitizer concentrations in the dishwasher. Facility does have quaternary strips for the surface sanitizer bottles.
    Correction: Provide a test kit or other device for measuring the concentration of all sanitizing solutions. Correct By: 24-Jul-2015

    Observation: Inside lip of the ice machine and the meat slicer on the left hand side of the prep table were observed visibly soiled.

    Observation: Inside of microwave is soiled.

    Observation: It was observed that eating utensils and plates are stored or displayed in dry storage area near the entrance of the facility where the food or lip contact surface could be contaminated. Utensils were in open boxes exposed to air drafts from vents and plates were stored eating side up.
    Correction: Store utensils in a manner to avoid contamination to the food and lip contact surface. Store plates inverted to avoid contamination to the food surface. Correct By: 24-Jul-2015

    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.

    Observation: Employee beverage was stored above food in the WIC
    Correction: Provide employee break room or designated area away from food preparation or storage for beverage storage. Correct By: 04-Oct-2013

    Observation: Employee noted working in food preparation/cooking area without a hair restraint.

    Observation: Pan of baked potatoes, green beans, and diced potatoes in the WIC were observed to be unwrapped or uncovered.
    Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
    Observation: The boxes of food in the dry storage area, boxes of food in the WIC and WIF are stored on the floor.
    Correction: Store all food items 6 inches above the floor. Correct By: 11-Oct-2013
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
    Observation: Boxes of single service items- cups, plates, lids are improperly stored on the floor in the main kitchen and back storage area.
    Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
    Observation: The plumbing associated with the handsink and ice machine is causes water to back up from the floor drain and puddle under the ice machine. Plumbing is not maintained in good repair.
    Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing. Plumber was contacted during inspection and will arrive soon. Correct By: 04-Oct-2013
  • LIGHT BULBS - PROTECTIVE SHIELDING (repeated violation)
    Observation: Lights located in dry storage area are not shatter resistant.
    Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs. Correct By: 11-Oct-2013

    Observation: No handwashing signage provided at designated handwashing sink.
    Correction: Provide handwashing signage at all handwashing sinks used by food employees. Correct By: 07-Oct-2013
