Observation: There are cut green peppers and pizza dough stored under raw hamburger in the pizza prep reach in cooler. There were shelled eggs over pickles in the walk-in cooler.
Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination. Raw meat was moved during the inspection.
- Critical: PHF/TCS, COLD HOLDING (corrected on site)
Observation: Cooked french fries were observed on the stove top cold held at 66°F.
Correction: Maintain cold potentially hazardous foods at or below 41°F. Fries were moved to the walk-in cooler during the inspection. Suggest a different technique for keeping fries such as portion the sizes or keep in a cooler.
- Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site)
Observation: Pork in the walk-in cooler was observed without a date marked. Corn salsa and other items in the True prep reach in cooler was observed with incorrect date marks.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. Items were marked with the correct dates during the inspection.

Observation: Ribs in the 2 door reach in cooler had exceeded its date mark of 5/23.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked. Ribs were discarded during the inspection.
- Critical: SEWAGE - BACKFLOW PREVENTION
Observation: A connection exists between the sewage system and ice bin drain located close to the kitchen.
Correction: Design the system in a manner that would preclude a connection between the sewage system and the drain. Remove the connection and provide a proper air gap. Correct By: 10-Jun-2015

Observation: There is no documentation of an approved variance for the pickled onions in the prep 1 door reach in cooler.
Correction: Submit a variance application to the regulatory authority for submission to the State for review. If approved, a copy of the approved variance shall be kept at the establishment and be subject to review by the inspector upon request. Discontinue current process until a variance can be obtained from the State. For now, the restaurant will only make small portions of pickled onions and keep for only 7 days. Correct By: 04-Jun-2015

Correction: Unapproved equipment shall be removed from food service. Existing Kenmore refrigerator can be used for the non-potentially hazardous food only, such as the items we discussed.
