    Observation: Ice making water is circulating across a heavy accumulation of slime and mold inside of the ice machine.
    Correction: Empty, disassemble, clean, and sanitize the ice machine according to the manufacturer's instructions. The ice machine must be cleaned and sanitized often enough to keep it free of mold and slime. The operator placed the ice machine out of service at the time of this inspection.
  • THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
    Observation: The backup cooler does not have a permanently affixed thermometer inside.
    Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device. Correct By: 19-Jan-2016

    Observation: The tile is missing around the area of the walk in cooler door. The crevice between the existing tile and the cooler wall has an accumulation of debris.
    Correction: Maintain the physical facilities so they are in good repair at all times. Provide a smooth level surface between the existing tile and the cooler wall and provide a smooth coved surface where the framed wall meets the cooler. Correct By: 23-Feb-2015

    Observation: Spray bottle with sanitizer used for food and non-food contact surfaces is not labeled with its contents.

    Observation: Seasoning used for baking breads are stored in unlabeled containers, all seasonings have similar appearence and texture.
    Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient.

    Observation: Beverage Cooler, Walk in Cooler, Norlake Two Door Cooler and 2 Door cooler under toaster oven are not equipped with an internal or permanently affixed temperature measuring device. Walk in Cooler and Norlake Two Door Cooler do have a readable external thermometer reading.
    Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device.
  • TOILET FACILITIES - RECEPTACLES - COVERED
    Observation: Female bathroom is not provided with a covered waste receptacle.
    Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.
  • FLOORS, WALLS AND CEILINGS - CLEANABILITY
    Observation: There is missing tile trim located by the entrance of the walk in cooler and mop sink area, wood and insulation is exposed. Also there is cracked tile at the entrance of the walk in cooler door
    Correction: dirt and other food debris is built up in this crack.
