IN-USE UTENSILS, BETWEEN USE STORAGE (corrected on site) Observation: The ice scoop in the ice bin below the Soda Dispenser was sitting with its handle touching the ice.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F. The ice scoop was placed into a corner handle up and out of the ice.
FOOD CONTACT SURFACES - ENCRUSTED ACCUMULATIONS Observation: The inside panel of the icemaker has some build up.
Correction: Clean and remove all encrusted soil accumulations from food contact surfaces of equipment.
DRYING MOPS Observation: Mops stored inside the mop bucket and the broom is stored on the floor.
Correction: Store mops to allow them to air dry without soiling walls, equipment and supplies. Store all cleaning gear on the mop hooks or on shelving.

Correction: Provide utensils with a handle for the ice bin. Store it handle up to prevent cross contaminating the ice.

Critical: PHF/TCS, COLD HOLDING (corrected on site) Observation: Milk in glass door beverage cooler is cold held at 52°F.
Correction: Maintain cold potentially hazardous foods at or below 41°F. Milk was removed from cooler and labeled "do not use" and placed on desk in office for owner. Operator observed cooler temperature to be above 41 this morning upon arrival and has been adjusting the temperature since. Strongly recommend discarding the milk that was stored in this cooler. If cooler is unable to maintain temps of 41 degrees and below, do not store potentially hazardous foods in this cooler.
Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED (corrected on site) Observation: Super San is not being used according to manufacturer’s use directions of 150-400 ppm quaternary ammonium. Tested 3 compartment sink to be 0-100 ppm.
Correction: Maintain 150-400 ppm quaternary ammonium as stated on Super San label. Operator drained and refilled sink. Sanitizer tested at 400 ppm.
WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES Observation: A chlorine test kit is not available for checking sanitizer concentrations for bleach used in wiping cloth buckets.
Correction: Provide a chlorine test kit or other device for measuring the concentration of sanitizing solutions.
PERSONAL CLEANLINESS - HAIR RESTRAINTS Observation: Employee noted working in food preparation/cooking area without an effective hair restraint. Visor is not effectively restraining long hair.
Correction: Provide employees working in food preparation and cooking areas with effective hair restraint (hat, hair net, beard restraint, etc.). Recommend long hair be tied back in addtion to using visor.
FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR (corrected on site) Observation: (REPEAT) Tomatoes are stored on the floor near office door.
Correction: Store all food items 6 inches above the floor. Operator relocated tomatoes.
