    Observation: Food employee observed improperly washing hands by turning off the water faucet with clean hands.
    Correction: Train food employees in proper handwashing techniques. Faucet should be turned off with single use paper towel so clean hands do not get contaminated from handles. Correct By: 24-Feb-2015
  • Critical: COMPLIANCE WITH FOOD LAW - UNAPPROVED SOURCE (corrected on site)
    Observation: Shelled eggs are being obtained from an unapproved source.
    Correction: Operator contacted vendor to obtain egg processor license number and will need to get a copy of the license to keep on-hand. Until proper license information is received, eggs will not be used in facility. If found on future inspections without license number, eggs will be discarded.
  • Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS (corrected on site)
    Observation: Frozen raw fish for thawing was observed being stored under raw chicken. Shelled eggs were observed being stored over salad dressing, relish, and other food items.
    Correction: Reorganize storage so that foods are stored above raw animal food and raw vegetables. Discard any ready-to-eat foods that show evidence of contamination. Operator moved raw fish above chicken. Shelled eggs were moved below salad dressing.

    Observation: The consumer advisory on the menu is missing an asterisk of symbol by each item that can be cooked to order.
    Correction: Provide an asterisk or symbol next to each item served in a raw or undercooked condition so that customers are directed to the consumer advisory. Correct By: 27-Feb-2015

    Observation: A quaternary test kit is not available for checking sanitizer concentrations.

    Observation: Meat slicer was observed visibly soiled. Ice machine was observed having water residue and mold build up next to the ice cube tray and had signs of buildup in the bin area.
    Correction: Maintain food contact surfaces in a clean condition. Ensure meat slicer is cleaned and sanitized before next use. Meat slicer was cleaned at end of inspection. Correct By: 26-Feb-2015

    Observation: No single-use toweling or other hand drying device available at bar area handwashing sink for hand drying.
    Correction: Provide single-use toweling or other approved devices at all handwashing sinks to facilitate proper handwashing. Correct By: 25-Feb-2015

    Observation: Working containers of quaternary sanitizer are not labeled with contents.

  • LABELING - FOOD STORAGE CONTAINERS LABELED WITH COMMON NAME (corrected on site)
    Observation: Various food items (flour, etc) are stored in unlabeled, reused containers in the back dry good storage area.
    Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient. Containers were labeled with contents.
  • WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER (corrected on site)
    Observation: Wiping cloth used for wiping counters, tables is stored on the side of the sanitizer container on floor.
    Correction: Cloths used for wiping counters, tables shall be stored in a sanitizing solution off of the floor. A container was made at the end of the inspection and wiping cloth was properly stored.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR (corrected on site)
    Observation: Mushrooms and thawing raw fish were observed being stored on the Traulsen 3 door cooler floor.
    Correction: Store all food items 6 inches above the floor. Mushrooms and fish were moved off of the cooler floor during the inspection.
  • EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS
    Observation: Observed two non-ANSI certified or approved by the department heating units in use for hot holding taco meat.
    Correction: Unapproved equipment shall be removed from food service. Replaced reheating, hot holding units and slow cookers with ANSI certified or department approved equipment. Correct By: 24-Jul-2015
  • SINGLE-USE AND SINGLE SERVICE ARTICLES - USE LIMITATIONS
    Observation: Commerically purchased bread bags were observed being reused to contain newly made fry bread.
    Correction: Discard single-use and single service articles after initial use. Correct By: 25-Feb-2015
  • TOILET FACILITIES - RECEPTACLES - COVERED
    Observation: Female bathroom is not provided with a covered waste receptacle.
    Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins. Correct By: 27-Feb-2015
  • INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS
    Observation: The exterior door in the back storage room entrance is not self-closing or tight-fitting.
    Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents. Correct By: 24-Apr-2015

    Observation: No handwashing signage provided at bar area handwashing sink.
    Correction: Provide handwashing signage at all handwashing sinks used by food employees. Correct By: 25-Feb-2015

    Observation: The condensor for the Traulsen three door cooler has visible dust. Areas have exposed insulation in back storage room area.
    Correction: Maintain the physical facilities so they are in good repair at all times. Clean cooler condensor to keep in good functioning condition. Cover exposed insulation. Correct By: 24-Apr-2015
