  • Critical: FOOD CONTACT SURFACES - WAREWASHING EQUIPMENT, CHEMICAL, TEMPERATURE, Ph, CONCENTRATION AND HARDNESS
    Observation: The sanitizer in the waitstaff dishmachine was found to be 0 ppm during the inspection. The container of santizer was empty. After changing the sanitizer container, the machine was primed 2 times, however the santizer concentration was still found to be 0 ppm.
    Correction: Repair the dishmachine so that it is properly dispensing the appropriate concentration of sanitizer. Until the machine is properly working, all glasses, utensils, etc must be sent through the kitchen dishmachine. The 4 compartment sink in the waitstaff area may be used for utensils, glasses, etc if the sink basins are cleaned out and set-up properly. Correct By: 13-Nov-2015
  • Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - DESIGN STANDARD
    Observation: There is no backflow preventor on the threaded faucet of the mop sink.
    Correction: Provide a State of Wisconsin approved backflow prevention device (ASSE 10/11). Correct By: 13-Nov-2015
  • DEMONSTRATION OF KNOWLEDGE - EMPLOYEE HEALTH
    Observation: Next to the handwashing sink in the back of the kitchen, there is a corporate employee health procedures guide posted. In this posting, it states employees who are ill with vomiting, diarrhea or fever must stay home for an additional 24 hours after symptoms are over. This policy does not match the Wisconsin Food Code requirements in regards to length of restriction of employees after experiencing symptoms of vomiting and diarrhea.
    Correction: The Wisconsin Food Code states employees must be excluded/restricted from the food establishment for a minimum of 48 hours after the food employee experienced their last symptom of vomitting or diarrhea. Inspector will forward information to Food Program Manager who reviews corporate policies and procedures. Correct By: 06-Dec-2015
  • USING A HANDWASHING SINK - ACCESSIBLE
    Observation: Handwashing sink nearest the walk-in freezer glass door was blocked by a blue wiping cloth bucket and a large garbage container during inspection and is unavailable for proper handwashing.
    Correction: Keep handwashing sink free of obstructions at all times to allow for frequent handwashing by employees. Correct By: 06-Nov-2015
  • HANDWASHING CLEANSER AVAILABILITY
    Observation: The soap dispenser is empty in the women's restroom.
    Correction: Provide hand soap at handwashing sink to facilitate proper handwashing. Correct By: 06-Nov-2015

    Observation: A chlorine test kit is not available for checking sanitizer concentrations at kitchen ECOLAB auto dishwasher.
    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions. Correct By: 06-Jan-2015
  • WAREWASHING EQUIPMENT - CLEANING FREQUENCY - AS NECESSARY TO PREVENT RECONTAMINATION
    Observation: The ECOLAB kitchen automatic dishwasher area(top panels, etc. are not cleaned at a frequency that prevents recontamination - soiled build up observed.
    Correction: Clean warewashing machine and components or the compartments of warewashing sinks at a frequency necessary to prevent recontamination of equipment and utensils. Correct By: 06-Jan-2015
