Correction: Raw fruits and vegetables shall be thoroughly washed in water to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption in READY−TO−EAT form. Always use the disposable gloves when cutting the limes and other fruit (ready-to-eat food).

Observation: Female bathroom is missing the lid to the waste receptacle.
Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.

Observation: Mop is stored in mop bucket with dirty mop water.
Correction: Store mops to allow them to air dry without soiling walls, equipment and supplies.

Observation: Quat sanitizer is not being used according to EPA registed label use instructions and is at <100 PPM.
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration.

Correction: Use covered tamper resistant bait stations for rodent bait.

Observation: A Quat sanitizer test kit is not available for checking sanitizer concentrations.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.

Observation: The ice machine bin has a biuild up on mold.
Correction: Clean utensil and equipment not contacting PHS/TSC food any time contamination occurs, at least every 24 hours, or at a frequency according to manufacturer specifications.

Observation: Single service articles suca as stir sticks are not available in the original intact wrapper or from an approved dispenser.
Correction: Single service articles shall be dispensed to consumers so they are protected from contamination.
