Observation: The silver ware container, walk in cooler, and saute pot handle have duct tape on them which makes them not smooth or easily cleanable to prevent contamination.
Correction: Discard equipment and utensils that are not: corrosion resistant, durable, able to withstand repeated warewashing, smooth, easily cleaning, nonabsorbent, and resistant to pitting, scratching, scoring or decomposition.
- FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED (repeated violation)
Observation: There is a tile/cove missing underneath the dish machine.
Correction: Provide base coving.
- LIGHT BULBS - PROTECTIVE SHIELDING
Observation: The light above the prep area by the walk in cooler has a hole in the cover.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.

- POSTING OF CERTIFICATE
Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate.
- IN-USE UTENSILS, BETWEEN USE STORAGE
Observation: Black portion control cups improperly stored in veggie broth mixture and oriental mixture. Scoop left lying inside ice bin.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The tile floor infront of the saute station has broken tiles and grout that has worn away allowing build up of food debris/residue between tiles.
Correction: Provide floors that are designed, constructed, and installed so they are smooth and easily cleanable.

Observation: Floor and wall junctures of the following areas are not coved with tiles missing or broken:
-under food prep sink
-behind saute station

Observation: The utility sink is broken with a crack on the surface not allowing cleanability and potential leaking of the sink.
Correction: Maintain the physical facilities so they are in good repair at all times.
