Observation: The sanitizer spray bottle was not being used according to EPA registered label use instructions and was at >200 PPM. Bleach was undiluted.
Correction: Provide training to employees on proper use and concentration of sanitizer and adjust to provide proper sanitizer concentration. Provide 50-100 ppm.
- CERTIFIED FOOD MANAGER – REQUIREMENT
Observation: There is no Wisconsin Certified Food Manager for this establishment.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within one month. Manager has taken the Serve Safe class but has not applied to the state for certification.

- HANDWASHING FACILITIES - NUMBERS AND CAPACITIES
Observation: Handwashing sinks are not provided separately from prewash, utensil washing, and toilet room handwashing sink. The prewash sink by the warewashing machine may not be used for handwashing. The three compartment sink in the kitchen may be used for washing, rinsing, and sanitzing utensils, or handwashing on the far left, unused middle compartment, and food preparation on the far right, but not both. A separate handwash sink may be required from the sink used with the toilet room.
Correction: Install approved handwashing sinks as required. Submit floor plan to the Department of Health Services for plan review to be completed by the Department. Correct By: 22-Sep-2014
