- Critical: PHF/TCS, COLD HOLDING (repeated violation)
Observation: Shredded lettuce, meat items, cheese on prep line are held at 68°F.
Correction: Maintain cold potentially hazardous foods at or below 41°F or have an approved time as a public health control plan in place.
- TIME AS A PUBLIC HEALTH CONTROL - PLAN APPROVAL AND PROPER METHODS
Observation: No time as a public health control plan is available for review or processes were observed that indicates a food was not properly prepared before using time as a public health control. Potentially hazardous foods/Time controlled for safety foods (PHF/TCS) (shredded lettuce, meat items, cheese, etc) sitting on prep line at room temperature (68F).
Correction: Food items out of temperature control shall be discarded and any food improperly prepared that is considered unsafe shall be discarded. Time as a public health control shall not be used until a plan is provided and readily available for review and approved by the regulatory authority.
- WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER (repeated violation)
Observation: Wiping cloth used for wiping counters stored all over prep areas of kitchen (counters, equipment, shelves, etc.).
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.
- EQUIPMENT AND UTENSILS - GOOD REPAIR
Observation: Walk In Freezer has excessive frost build up all over shelves, food items, floor, and ceiling - door is not able to be properly closed.
Correction: Repair equipment to good condition.
- NON-FOOD CONTACT SURFACES - CLEAN
Observation: Non-food contact surfaces of soda machine have mold growth in several areas.
Correction: Ensure equipment is cleaned and sanitized and remove dirt and mold from all non-food contact surfaces of equipment.

Observation: Raw eggs cold holding at 61°F on counter top and cut lettuce and spring mix cold holding on counter top at 51°F.
Correction: Maintain cold potentially hazardous foods at or below 41°F or follow McDonald's Corporate policy recently approved by the state of Wisconsin.
- WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER
Observation: Wiping cloth used for wiping counters stored on counter tops.
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.
- EQUIPMENT AND UTENSILS - FIXED , SPACING OR SEALING - INSTALLATION
Observation: Handsink is fixed in place, but not properly sealed to allow for easy cleaning.
Correction: Handsink must be re-grouted to allow for proper cleaning and so that it is smooth and non-pourus.
- HANDWASHING SINKS, INSTALLATION - NON-HAND OPERATED
Observation: Handwashing sink faucet is not adjusted properly so that it is non-hand operated.
Correction: Repair faucet handle to ensure it is non-hand operated..
- DRYING MOPS
Observation: Mops stored in mop bucket.
Correction: Store mops to allow them to air dry without soiling walls, equipment and supplies.
