- WIPING CLOTHS - USE LIMITATION - SANITIZING SOLUTIONS (repeated violation)
Observation: I checked saniizing buckets for both quat and chlorine sanitizer and detected 0ppm.
Correction: Provide proper concetration in sanitizing buckets for wiping cloths.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: There is an area missing cove tile near the office.
Correction: Replace missing cove tile. Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.

- Critical: PLUMBING - REPAIRED TO UNIFORM PLUMBING CODE
Observation: The overhead hand sprayer at the 4 compartment sink leaks and was repaired with plastic bag around it. Also the left side faucet is missing the cold water handle. Also the right side faucet drips and does not shut off completely.
Correction: Repair the hand sprayer, replace the missing faucet handle and repair the drip.
- WAREWASHING - CHEMICAL SANITIZER CONCENTRATION
Observation: The Ecolab dish machine was not dispensing any sanitizer in final rinse. The soap was empty. No test kit is available for monitoring sanitizer concentration. Without soap and proper sanitizer utensils are not being washed and sanitized properly.
Correction: Repair dish machine to provide a minimum of 50ppm chlorine in final rinse. Also provide soap in wash cycle. Provide a test kit to allow accurate determination of the sanitization concentration. This machine must be tested once per shift and a log sheet maintained to assure the machine is functioning properly.
- HAND DRYING PROVISION (repeated violation)
Observation: The paper towel dispenser near the grill area is not operational (needs batteries).
Correction: Repair dispenser so there are paper towels available at all times.
- POSTING OF CERTIFICATE
Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate.
- IN-USE UTENSILS, BETWEEN USE STORAGE
Observation: The ice scoop inside the ice bin near the drive-thru window was laying on ice with handle contacting ice.
Correction: Store scoop with handle out of ice.

Observation: Creamer in the Sure Shot cooler was 47 degrees..
Correction: Maintain cold potentially hazardous foods at or below 41°F
- Critical: TOXIC SUBSTANCES - SEPARATION
Observation: There was a spray bottle of degreaser stored next to two spray bottles of cooking spray (next to grill). Also a spray bottle of stainless steel cleaner was on top of freezer unit (next to grill).
Correction: Keep all chemicals, cleaners, etc. stored seperate from food or food prep areas.
- HAND DRYING PROVISION
Observation: There was no paper towels available at either of the two hand wash sinks in the kitchen. (paper towel was put in one of the dispensers while I was doing the inspection)
Correction: Provide paper towels at all times at the had wash sinks.
- EQUIPMENT AND UTENSILS - NONFOOD CONTACT SURFACES - CONSTRUCTED
Observation: Racks inside of the small cooler (east wall of kitchen) are rusted. Also clean the inside of this cooler and the upright cooler in the center of the kitchen. Additionally, there is a dirty cardboard box (with saran wrap) that is right above the brewed tea.
Correction: Provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials.....move saran wrap or place it in a different box that isn't dirty and falling apart.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: There are several small holes in the west kitchen wall.
Correction: Fill all of the small holes in the wall(s).
