Observation: The deli display case glass is cracked and duct tape is used on sliding doors. Styrafoam exposure on small chest freezer must be repaired or replaced to allow for proper cleaning. Large chest freezer no longer closes on its own and is held closed with paint cans due to broken seal.
Correction: Repair equipment to good condition or remove from premise.

Observation: Equipment surfaces (including mixers, bread slicer, cooler doors) were found to be visibly soiled with food debris and dirt.
