- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: The following are in disrepair and are not easily cleanable:
-Tile grout is missing in several areas of the kitchen (deep frier area, dish area, etc.). Tile grout is missing between floor and cove base in the dish/mop area.
-Floor tiles are missing in the deep frier area.
-Sides of walk-in cooler noted in disrepair.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
- CLEANING, FREQUENCY AND RESTRICTIONS
Observation: The following areas noted in need of cleaning:
-Floor in the corner by the filtration system
-Wall behind and underneath 3-compartment sink
-Floor and surfaces underneath drive thru soda machine
-Behind and underneath main line equipment
-Walk-In Cooler shelving, floor, and walls
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

- NON-FOOD CONTACT SURFACES - CLEAN
Observation: The following non-food contact surfaces noted in need of cleaning:
-Floor drains throughout kitchen area
-Corner area by filtration system
-Wall behind and underneath 3-compartment sink
-Floor and surfaces underneath drive thru soda machine
-Both soda machine dump trays and underneath by nozzles
-Behind and underneath main line equipment
-Walk-In Cooler shelving, floor, and walls
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: The following are in disrepair and are not easily cleanable:
-Tile grout throughout the kitchen is missing. Regrout floor and tile cove base throughout kitchen.
-Floor tiles and tile cove base are missing in several areas of the kitchen. Replace/repair all broken and missing tile.
-Walk-In Cooler and Walk-In Freezer doors are in disrepair. Replace/repair both walk-in cooler and freezer doors so that they are easily cleanable.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
Observation: There is a gap in the drive thru window door which could allow the entrance of insects or rodents.
Correction: Repair this hole or gap so that entrance by insects or rodents into the food establishment is prevented.

Observation: Observed ice scoop stored on top fo ice machine. Store ice scoop in protected container.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
- NON-FOOD CONTACT SURFACES - CLEANING FREQUENCY
Observation: Observed floor of walk in freezer soiled. Clean the floor of walk in freezer as often as needed.
Correction: Clean all equipment on a routine basis to prevent accumulation of soil residues.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: Observed wall underneath the (4)-compartment sink with hole. Repair tile work/ wall to maintain wall as easily cleanable.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
