    Observation: There are ready-to-eat rice and various other foods stored under raw eggs in the walk in cooler. Raw fish was observed stored over pickles in the walk-in cooler. Raw chicken was stored over shelled eggs in the walk-in cooler.
    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination. Foods were moved during the inspection.
  • Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER (corrected on site)
    Observation: Raw meats were stored in same container or stacked atop one another in same container in the walk-in cooler.
    Correction: Prevent nesting of raw animal foods and store raw animal foods in order of decending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients. Meats were separated during the inspection.

    Observation: Pancakes in a back hot holding unit were observed hot held at 128.0°F
    Correction: Maintain hot potentially hazardous foods at or above 135°F. Pancakes were discarded during the inspection.
  • Critical: PHF/TCS, COLD HOLDING (corrected on site)
    Observation: Mushrooms, sliced tomatoes, ham and chicken were observed cold held above 41°F during the inspection in the True 2 door reach in cooler near the grill.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. Ice was added to the food items during the inspection.
  • Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site)
    Observation: Sausage gravy, chicken enchilada sauce, cooked rice, quiche, rotary mc rib, cooked noodles, cooked dressing, cooked beans, and other items were observed without date marks in the walk-in cooler.
    Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. Date marks were added during the inspection.
  • Critical: DATE MARKING - DISPOSITION (corrected on site)
    Observation: Taco meat was observed in the walk-in cooler that had exceeded its date mark of 10/13.
    Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked. Taco meat was discarded during the inspection.
  • Critical: SEWAGE - BACKFLOW PREVENTION
    Observation: A connection exists between the sewage system and ice machine and all ice bin drains. Drains have an air break.
    Correction: Design the system in a manner that would preclude a connection between the sewage system and the drains. Remove the connection or air break with a proper air gap. Correct By: 20-Nov-2015
  • Critical: TOXIC SUBSTANCES - STORAGE (corrected on site)
    Observation: Various items such as Gorilla glue, hydrogen peroxide and rubbing alcohol were observed stored over potatoes and other foods in the dry food storage area.
    Correction: Rearrange area so that toxic materials are stored below or away from food, equipment and single service items. All toxic items were moved to the chemical storage area.
  • Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED
    Observation: Quaternary and bleach sanitizers were observed not being used according to manufacturer’s use directions. Quaternary solution was over 500ppm and bleach solution was 0ppm during inspection. It was also noted that dish soap was being added to the bleach solution.
    Correction: Change procedures and provide training to employees on proper use of toxic chemical following manufacturer's directions for use. Buckets were emptied and remade during the inspection. Quaternary was 400ppm and bleach was 50ppm. No soap was added to buckets.

    Observation: The consumer advisory is provided on the menu, but is missing the symbol next to each corresponding item.

    Observation: Taco meat in the walk-in freezer and raw fish in the walk in cooler were observed to be unwrapped or uncovered.
    Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods. Covers were added during the inspection.
  • WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER (repeated violation)
    Observation: Wiping cloth used for wiping counters stored on the sides of the waitstaff bucket.
    Correction: Cloths used for wiping counters shall be stored in a sanitizing solution. Correct By: 30-Oct-2015

    Observation: There are hot dog buns and apple turn overs stored on the floor in the walk in freezer.

    Observation: There are a variety of items such as dressings, radishes, pickles, bacon bits and more are available in uncovered containers or is otherwise not protected from contamination in consumer self-service area.

    Observation: Food items were observed subject to potential contamination by storage of uncovered employee drinks in the dry goods area and employee items such as coats hung on the food rack.

    Observation: Non-food contact surfaces of soda gun holders in the back bar were observed soiled with debris and yeast.
    Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. Holders were removed during the inspection, washed and sanitized.

    Observation: It was observed that eating utensils are stored or displayed in blue bins in the front kitchen or waitstaff area where the food or lip contact surface could be contaminated.
