Observation: Gyros stored on the bottom cooler drawer were stored at 47°F. Items stored on the cookline cooler well were holding at 45F.
Correction: Maintain cold potentially hazardous foods at or below 41°F. Gyros portion was discarded. It is my recommendation that items stored in the cookline cooler well should use time as a public health control. Management agreed and will discard all at 3 PM.
- USING A HANDWASHING SINK - ACCESSIBLE
Observation: Handwashing sink was not operational and is unavailable for proper handwashing.
Correction: Keep handwashing sink free of obstructions and in working condition at all times to allow for frequent handwashing. Hands free control module broke on 8/11/15 and new part is on order and should be repaired by 8/15/15.
- EQUIPMENT AND UTENSILS - GOOD REPAIR
Observation: Cookline freezer door handle is broken.
Correction: Repair equipment to good condition or remove from premise.

Correction: Provide approved beverage container in the food preparation area.

Observation: The "Veggie Freshener" was stored in an unlabeled container.
Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient.

Observation: No handwashing signage was provided at the warewashing area handwashing sink.
Correction: Provide handwashing signage at all handwashing sinks used by food employees.

Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.

Observation: There is no Wisconsin Certified Food Manager for this establishment.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days. Correct By: 25-Dec-2014

Observation: Improper storage of in use utensils in between use. Dirty utensils were stored in the dipper well and water was not running.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
