Observation: Customer's pre-set silverware obseved wrapped by employee where eating ends of the silverware are touched by employee when wrapping with outer paper cover.
Correction: Clean food contact surfaces anytime contamination has occurred. Employee should wear rubber gloves or touch only handle end when wrapping pre-set tableware. Correct By: 25-Apr-2015

Observation: Drinking cup was observed in dining area on shelf directly above ice machine and directly above clean stacked dishes at 4 compartment sink.

Observation: The whole pineapples in produce box observed stored on the floor in the back kitchen area.
Correction: Store all food items 6 inches above the floor. Correct By: 25-Apr-2015

Observation: Frigidaire combination cooler/freezer is used to store PHFs and is not ANSI certified or approved by the department.
Correction: Unapproved equipment shall be removed from food service. Use only to store non-PHF foods as explained or replace with ANSI food approved refrigerator/cooler. Correct By: 25-May-2015

Observation: Leaking drain plug hole in 3rd compartment sink observed at 4 compartment sink. Plumber/contractor called. Will use auto dishwasher unitl repaired.
Correction: Items that are unnecessary to the operation of the food establishment shall be removed from the premise. Correct By: 25-Mar-2015
