    Observation: Food employee observed improperly washing hands by turning off the facet with clean hands.
    Correction: Train food employees in proper handwashing techniques. Review that disposable towels or at a minimum the wrist should be used to turn off the faucet. Correct By: 10-Nov-2015
  • Critical: PHF/TCS, HOT HOLDING (corrected on site)
    Observation: Hamburger in front display case was observed hot held at 132.8°F.
    Correction: Maintain hot potentially hazardous foods at or above 135°F. Temperature was turned up on the unit and hamburger was temped again at 136.8F.
  • Critical: TOXIC SUBSTANCES - STORAGE (corrected on site)
    Observation: Chemical bottle was observed stored on counter right next to open container of pizza sauce in the front pizza preparation area.
    Correction: Rearrange area so that toxic materials are stored below or away from food, equipment and single service items. Bottle was moved from location during the inspection.
  • Critical: TOXIC SUBSTANCES - CONDITIONS OF USE - USED (corrected on site)
    Observation: Multi-quaternary solution was observed not being used according to manufacturer’s use directions at the time of inspection. Bottle registered 0ppm.
    Correction: Change procedures and provide training to employees on proper use of toxic chemical following manufacturer's directions for use. Bottle was emptied and refilled to register 400ppm during the inspection.
  • TOXIC SUBSTANCES - IDENTIFYING INFORMATION, PROMINENCE - COMMON NAME-WORKING CONTAINER (corrected on site)
    Observation: Working containers of Multi-Quat sanitizer were observed not labeled with contents.
    Correction: Label working containers. Containers were labeled during the inspection.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
    Observation: There was coffee observed stored on the floor in the kitchen and several food items stored on the floor in the walk-in freezer.
    Correction: Store all food items 6 inches above the floor. Correct By: 13-Nov-2015
  • NON-FOOD CONTACT SURFACES - CLEAN
    Observation: Non-food contact surfaces such as the doors, gaskets, and insides of the two reach in coolers were observed soiled with food debris.
    Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. Correct By: 13-Nov-2015
  • HANDWASHING SINKS, INSTALLATION - NON-HAND OPERATED
    Observation: Handwashing sink in the kitchen is not non-hand operated. Observed sink to have small, turn knob handles.
    Correction: Employee handwashing sinks shall be non-hand operated and if metered shall provide a flow of water without the need for reactiviation for at least 15 seconds. Correct By: 01-Jan-2016

    Observation: The floor in the kitchen and front service areas were observed to have broken and missing tiles.
    Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable. Correct By: 01-Jan-2016

    Observation: Floor and wall junctures in the kitchen area near the stove and grill were observed to not be coved or sealed.
    Correction: Provide base coving. Correct By: 01-Jan-2016

  • Critical: REHEATING FOR HOT HOLDING (corrected on site)
    Observation: Taco meat, pork, soup was improperly reheated, placed in the hot hold unit to be reheated and soup was in a self service kettle while being reheated. Temperatures for the products listed were found to be 105 to 122 degrees F during the one hour time frame it was monitored.
    Correction: Adjust procedures and methods to properly reheat food. Commercially processed foods must be reheated to a temperature of 135 degrees F or above quickly and before service to the public the first time and subsequent times it must be reheated to 165 degrees F. If your unit is both equipped to reheat and hot hold then reheat the food quickly and adjust the temperature to then hot hold. Soup that is available for self service must be labeled if it is not ready to eat but still out in the customer area. Another method would be to reheat all food in the kitchen to the proper temperature before placing on the line.
  • Critical: FOOD CONTACT SURFACES - SANITIZED BEFORE USE AFTER CLEANING (corrected on site)
    Observation: All dishes & equipment being run thru the mechanical dishwasher are being properly sanitized after cleaning.
    Correction: Clean and sanitize all utensils and equipment before contact with food.
  • WAREWASHING MACHINES - AUTOMATIC DISPENSING OF DETERGENT AND SANITIZERS (corrected on site)
    Observation: The warewashing machine does not dispense detergent or sanitizer automatically. Test strip did not register any concentration of sanitizer.
    Correction: Repair or replace the warewashing machine. Warewashing machines shall automatically dispense detergents and sanitizers. I thought the tub was being pinched by the full box sitting on top of it - that is not a good practice. Operator primed the sanitizer tube to remove air in line.
  • HANDWASHING CLEANSER AVAILABILITY
    Observation: Clearly marked soap is not available at food employee handwashing sink. The designated soap dispensesr is empty. There is a dispenser labeled foaming sanitizer next to it, employee is sure that is soap and not sanitizer.
    Correction: Provide hand soap at handwashing sink to facilitate proper handwashing. Properly label the soap dispenser and remove dispensers that are not in use. Hand soap must be provided, sanitizer is not required. Sanitizer label was blacked out during inspection.

I know way too many people who have had food poisoning from this joint. It's horrible.
