Observation: There are ready-to-eat lettuce, potatoes, and shelled eggs were observed stored under raw hamburger patties in the Frigidaire glass cooler.
Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination. Food items were rearranged so that the raw hamburger was on the bottom shelf.

Observation: Containers of taco meat were observed without date marks in the chest freezer upstairs and various food items were without date marks in an upright freezer in the basment.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. Date marks were added during the inspection.

Observation: The consumer advisory is provided on the menu but is missing a symbol or asterisk next to it and each corresponding food item.
Correction: Provide a symbol or asterisk next to the consumer advisory and each animal food served in a raw or undercooked condition. Correct By: 22-Jan-2016
- THERMOMETERS - FOOD - ACCURACY
Observation: Thermometer used in food establishment is not accurate to +/- 8°F. Observed two thin probe thermometers in establishment reading 70°F and 78°F. Both thermometers are in need of recalibration.

Observation: Onions were observed stored on the floor in the basement.

Observation: Female bathroom is not provided with a covered waste receptacle.
Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins. Correct By: 22-Jan-2016

Observation: No handwashing signage provided at kitchen handwashing sink.
Correction: Provide handwashing signage at all handwashing sinks used by food employees. Correct By: 15-Jan-2016

Observation: Mechanical dishwasher is not ANSI certified or approved by the department. Sticker is NSF - Residential
Correction: Unapproved equipment shall be removed from food service. Replace the mechanical dishwaser with a commercial mechanical dishwasher or use the available 4 compartment sink for wash - rinse - sanitizing of dishes. This is a repeat violation and if it is noted at the next inspection it will trigger a reinspection at the facility. Correct By: 05-Mar-2015
