  • Critical: PREVENTING CONTAMINATION FROM HANDS - NO BARE HAND CONTACT WITH RTE-FOODS (corrected on site)
    Observation: Employee observed handling ready-to-eat sandwiches and lettuce with bare hands.
    Correction: Employees shall use single-use gloves, utensils, bakery papers or other approved means when handling ready-to-eat foods. Gloves subsequently used. Gloves must be changed and hands washed when changing tasks such as moving from handling raw animal food to ready-to-eat food. Infected food employees are the source of contamination in approximately one in five foodborne disease outbreaks reported in the United States with a bacterial or viral cause. Even though bare hands should never contact exposed, ready-to-eat food, thorough handwashing is important in keeping gloves or other utensils from becoming vehicles for transferring microbes to the food. Having a high potential contamination level on the hands combined with a very low infectious dose necessary to cause infection are the reasons that FDA believes that handwashing alone is not an effective single barrier in the transmission of these fecal-oral pathogens.

    Observation: Wiping cloth in sanitizer by prep cooler table is above 200 ppm chlorine.
    Correction: Sanitizer shall meet the requirements under 40 CFR 180.940. Operator refilled bucket with 300 ppm quaternary ammonia.

    Observation: Improper cooling methods are observed for mashed potatoes at 44.6F cooling since yesterday.
    Correction: Adjust methods or procedures to facilitate cooling as described in 3-501.15. Mashed potatoes discarded.
  • HAND DRYING PROVISION (repeated violation)
    Observation: No single-use toweling or other hand drying device available at handwashing sink for hand drying in the kitchen.
    Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing. Correct By: 11-Feb-2016

    Observation: Employee noted wearing bracelets on wrist while working with food.

    Correction: Provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials. At a minimum, finish shelving with a medium gloss paint. Recommended lamiante or more durable material. Start with areas most prone to spills. Correct By: 28-Apr-2016

    Observation: Roast beef on top of food pan on prep cooler top is cold held at 60.1°F. Cooked pork in the prep cooler cabinet is at 46.4F. Other foods and air temperature in this cooler are 41F and below.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. Store foods in top of prep cooler below the tops of the food pans. Limit the amount of time food is removed from the prep cooler cabinet. Roast beef and cooked pork discarded during the inspection.
  • WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES
    Observation: A quaternary ammonia test kit is not available for checking sanitizer concentrations.
    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions. Correct By: 13-Nov-2014
  • HAND DRYING PROVISION (corrected on site)
    Observation: Paper towel dispenser for handwash sink by prep table is empty.
    Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing. Operator restocked paper towel dispenser and provided cloth towels for single use during inspection.
  • WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER (corrected on site)
    Observation: Damp wiping cloth used for wiping counters stored on prep cooler cutting board.
    Correction: Cloths used for wiping counters shall be stored in a sanitizing solution. Damp cloth sent to soiled linens and a clean cloth put in 400 ppm quaternary ammonia sanitizer.
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION (corrected on site)
    Observation: Knives are stored between prep cooler and steam table. Food residue has built up and is not being cleaned every four hours as required for food contact surfaces in use.
    Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor. Do not store knives between prep cooler and steam table. Operator put knives on cutting board during inspection. Recommended knives be placed in a bucket of sanitizer or on a magnetic knife strip which is easily accessible and cleanable.
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
    Observation: The handwashing sink is not maintained in good repair. Operator indicated a defective valve is on order. Bar area handwashing sink is available.
    Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing. Correct By: 14-Nov-2014
