**CDC Risk Factor Violation 14 F**:
• Areas noted in need of cleaning; clean plastic storage containers in clean storage (washed during the inspection), inside ice maker,
outside of bulk salad dispenser and large sink covers.
4-601.11 Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils.
(A) EQUIPMENT FOOD-CONTACT SURFACES and UTENSILS shall be clean to sight and touch.
(B) The FOOD-CONTACT SURFACES of cooking EQUIPMENT and pans shall be kept free of
encrusted grease deposits and other soil accumulations.
(C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of
dust, dirt, FOOD residue, and other debris.
**CDC Risk Factor Violation 17 A**:
• Chili in the wait staff steam table was at 116f (reheated during the inspection)
3-403.11 Reheating for Hot Holding.
(A) Except as specified under ¶¶ (B), (C), and (E), POTENTIALLY HAZARDOUS FOOD that is
cooked, cooled, and reheated for hot holding shall be reheated so that all parts of the FOOD
reach a temperature of at least 165 degrees F for 15 seconds.
(B) Except as specified under ¶ (C), POTENTIALLY HAZARDOUS FOOD reheated in a microwave oven for hot holding shall be reheated so that all parts
of the FOOD reach a temperature of at least 165 degrees F and the FOOD is rotated or stirred, covered, and allowed to stand covered for 2
minutes after reheating.
(C) READY-TO-EAT FOOD taken from a commercially PROCESSED, HERMETICALLY SEALED CONTAINER, or from an intact PACKAGE from a FOOD PROCESSING PLANT
that is inspected by the FOOD REGULATORY AUTHORITY that has jurisdiction over the plant, shall be heated to a temperature of at least 135
degrees F for hot holding.
(D) Reheating for hot holding shall be done rapidly and the time the FOOD is between the
temperature specified under Subparagraph 3-501.16(A)(2) and 165 degrees F may not
exceed 2 hours.
(E) Remaining unsliced portions of roasts of beef that are cooked as specified under ¶
3-401.11(B) may be reheated for hot holding using the minimum time and temperature conditions specified under ¶ 3-401.11(B).
**Critical Violation 26 D** :
• A container of motor oil was found stored with equipment. Removed during inspection.
7-202.11 Restriction.
(A) Only those POISONOUS OR TOXIC MATERIALS that are required for the operation and maintenance of a FOOD ESTABLISHMENT, such as for the
cleaning and SANITIZING of EQUIPMENT and UTENSILS and the control of insects and rodents, shall be allowed in a FOOD ESTABLISHMENT.
(B) Paragraph (A) does not apply to PACKAGED POISONOUS OR TOXIC MATERIALS that are for retail sale.
Violation 54 G:
• Aprons were found stored on bags of flour and employee sweat shirts hung on food storage racks in the dry storage area.
6-501.110 Using Dressing Rooms and Lockers.
(A) Dressing rooms shall be used by EMPLOYEES if the EMPLOYEES regularly change their
clothes in the establishment.
(B) Lockers or other suitable facilities shall be used for the orderly storage of EMPLOYEE
clothing and other possessions.
Out Department received a call from a lady who ate on 3-26-10 at a restaurant in Greenfield (reported to the Greenfield Health Department) and
at this establishment on 3-27-10. She became ill late in the p.m. on 3-28-10, saw a Doctor on 4-1-10 and tested positive for Salmonella. She
reports having a rib-eye steak, mushrooms and chili at this establishment..
At the time of this visit the complaint was discussed with the G.M. and a complete inspection was made.
1. The G.M. states that to his knowledge, no employees were sick during the 2 weeks prior to 3-27-10. The restaurant has a written employee
health policy that is posted on the employee bulletin board.
2. During this visit, I observed good hand washing and glove use.
3. Steaks are cut daily in the meat cooler and stored in the grill drawers until preparation for order. (Walk-in temperature today - 32f,
Grill drawer temperature - 39f).
4. Mushrooms are sautéed in 2 lb patches thru out the day and held until service. Leftovers are disposed of. (recorded holding temperatures-
164f & 171f)
5. Chili is made in the restaurant (policy is a 2-day shelf life), cooled in small containers in ice bath and walk-in (recorded cold holding -
40f), reheated and held hot until service ( recorded wait staff temperature -116f, reheated during the inspection).
Recorded 100ppm of sanitizer in the final rinse of the low temperature dish machine.
Recorded 200ppm of Oasis 146 sanitizer in the final rinse of the 3-compartment sink.

This is follow-up to my inspection of 4-6-10. Cleaning of food contact surfaces has improved (i.e., sink covers and inside of ice maker). The
ice maker is scheduled to be deep cleaned this week.
