  • Observed three dented cans in the same cabinet in the estates kitchen. All cans with dents on the seam must be discarded or given back to the supplier. Ensure all employees that stock items are properly removing dented cans from service. Manager discarded cans during inspection.
  • The small refrigerator located in the estates kitchen was reading between 47F-49F. Manager discarded all potentially hazardous foods immediately, and lowered the temperature of the refrigerator. He stated he will keep it closed until 3:45 to ensure the refrigerator holds temperature. Ensure that all coolers are maintaining 41F. Provide TCHD with 2 weeks of temperature documentation to ensure that the refrigerator is holding temperature by 2/22/16.
  • Observed the following food contact surfaces in need of repair: masking tape used to hold Vulcan oven handle together, damaged counter tops in both kitchens, damaged counter by can opener. Repair to allow for proper cleaning.
  • Observed untreated wood used to hold up shelving in the standing refrigerator in the dry storage room. Untreated wood is not approved for use in food service as it is not easily cleanable and absorbent. Repair or replace.
  • Observed the seal on the back of the hand sink to be damaged and peeling away. Repair or replace to allow for proper cleaning and prevent debris build up.
  • Observed a water stain underneath the sink in the estates kitchen that has caused for paint and pieces of woods to peel up. Repair or replace to allow for proper cleaning.
  • Observed no sanitizer present in the sanitizer bucket located in the dish room. All sanitizer buckets must have a concentration of at least twice the strength of the 3 compartment sink. Employee removed the bucket from service and remade it during inspection.
  • The 3 compartment sink in the big kitchen registered less than 100 ppm Quaternary Ammonia. Quaternary Ammonia must be between 150-400 ppm. The employee stated she just made the sink up and it registered fine. Manager pressed button and the ppm was brought up to 150 ppm. Ensure employees are verifying concentration and have maintenance done to ensure the machine is working correctly.
  • Observed the following areas of the floors in need of repair: cracked tiles by ice machine in estates dining room, damaged floor and coving behind ice machine in estates dining room, and a gap where a seal is missing where the floor meets the walk-in refrigerator in the big kitchen. Repair or replace to allow for proper cleaning.
  • Observed build up of debris along the corners of the dry storage room. Ensure employees are cleaning the floors thoroughly on a routine basis.

  • Observed build up around equipment in main kitchen. Wash, rinse, and sanitize.

  • The window frame around the window by the 3-compartment sink is chipping. Repaint window frame. The door to the walk-in has dents and insulation is beginning to show. Repair the door.
  • The concentration of the sanitizer buckets was low ~100ppm Quat. Buckets were remade to between 200-300ppm.
  • Observed build-up on ice machine drip tray. Wash, rinse, and sanitize on a daily basis.
  • All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the outside of the ovens and the water/ice machine grate in the satellite kitchen.
  • Found build-up of debris on floors. Clean the floors.
  • Observed dust build up and peeling paint on ceiling vents. Clean and repaint vents and maintain.

  • All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to fans, the back of the stove, the outside of the dish machine, and the water/ice machine grates in the satellite kitchens.
  • Observed dust/dirt debris on floors especially under equipment and in corners. Clean the floors.
  • The seal on the handsink near the dish machine is broken and crumbling. Reseal sink to the wall.
  • Observed food splash and debris on some of the walls. Clean the walls.
  • Observed dust build up on the ceiling vents. Clean and maintain the vents.

This place gets no stars!!! Look at the dates when the last reviews were taken! This is Sept of 2015. I'll begin by saying the nursing skills R a trocious!!! My mom has laid in her own urine for 6 hours at a time, a pill was found under my moms bed, there was no hot water, her meds were not given correctly, the administrator lied to our faces, the CNA's r untrained, the list goes on, so I'd really think twice about this facility for ur loved one. There is no communication skills whatsoever! Oh and also a LPN who was a young man Dane in to treat my moms MRSA, used the tube up her nose instead of a nose swab and my sad said what in the hell R u doing? He said oh i forgot the swabs. Really??? Also my mom was given a pill she was highly allergic to and the nurse said oh I guess I should have looked at the note! Again extremely poor care,, my mom is laying there dying because they don't give a shit! Medicare is a joke too! So do ur homework and also b prepared to b ur loved ones advocate!
