- Observed potatoes in a grey container with an empty cardboard box with slight water stains placed on top in the inside. Facility must not place food containers on top of other foods in containers. Facility must provide covering for all foods that are in storage, except when in use. Facility removed cardboard box and provided a cover during inspection.
- Observed the following food contact surfaces in need of repair: paint peeling on shelving above drop in cooler and tape used to repair cracks in the ice cream cooler shield. Food contact surfaces must be smooth, easily cleanable, and in good repair. Tape is not an approved material for food service. Repair these to allow for proper cleaning.
- Observed paper towels placed in the drain located on the soft-serve ice cream machine. All food contact surfaces must be non-absorbent and easily cleanable. Paper towels create an absorbent surface that is not easily cleanable. Facility should allow the dripping ice cream to land on the plastic collection plate and metal grate below then wash, rinse, and sanitize as routinely as necessary. Manager removed towels during inspection.
- Observed build up of debris on equipment and pipes located behind the cooking line and under the customer soda machine. Clean as routinely as necessary.
- Observed three small leaks on the 3 compartment sink faucet: the swing joint, the faucet head, and along the water hook up to faucet. All plumbing must be maintained in good repair. Repair.
- Observed a large amount of damage to the bluish-grey floor in the dry storage area and cracked tiles in the corner by 3 compartment sink and single sink. All floors must be smooth, free of cracks, easily cleanable, and in good repair. Repair all cracks and crevices to allow for proper cleaning.
- Observed build up of debris on floor underneath the customer soda machine. Clean as routinely as necessary.
- Observed the following damage to walls and ceilings throughout facility: hole in the wall by the canned goods rack, the seal provided where the walls meet in the dry storage area by canned goods is beginning to break, coving is becoming damaged by mop sink allowing for debris build up, the hand sink & single sink should be sealed to wall, missing wall covering over the area where pipe was repaired under the prep table, wall around back exit door has a gap (no entrance from outside), and there are gaps around both pipes exiting the wall by the ice cream. Repair, reseal, or replace to allow for proper cleaning.

- Observed broken floor tiles near grill and near back handsink. Replace tiles.
- Observed a hole in the wall under the handsink near the back door. Fix the hole.
- Observed mop stored in used water in the bucket. Store mop so it can air dry. Mop can be stored in the grated portion of the bucket.

- Observed broken door seal gasket on reach in freezer by dry storage. Observed tape on the handle of the reach in freezer near soda lines.
- Found build-up on handles of coolers, freezers, microwaves, and soft serve machine. All non-food contact surfaces should be washed, rinsed, and sanitized.
- Broken floor tiles in the grill area. Repair or replace.
- Build-up on the floor under equipment especially around the legs. Clean the floors.
- Ceiling wall junction seal is chipped away and wood is showing through near dry storage.
- Build up on wall behind 3-compartment sink and along the wall ceiling junction along the red tile. Clean the walls.
- Observed a mop in the bottom of the bucket with water in it. Store mop so that it may air dry.
