- There is heavy frost buildup over all the boxes in the walk in freezer. Per manager, there was a pipe leak in the unit. Maintenance has ordered a new part, which they are waiting on.
- Observed unsealed wood shelves in deli room. Re-paint or otherwise seal shelves. Observed damaged gaskets in meat room walk in cooler. Replace gaskets. Observed shelves in meat room walk in cooler lined with paper. Also observed cardboard being used as a shelf liner under 24-packs of water in back hallway. Ensure all surfaces or liners are nonabsorbent, easily cleanable, smooth, and durable. Vents in both walk in coolers are heavily rusting. Resurface or replace. Self-latching device on walk in freezer is broken. Repair.
- Observed food buildup in microwave in deli area. Routinely clean the inside of the microwave.
- Observed damaged floor tiles throughout the facility, particularly near front registers. Repair floors.
- Observed small holes and several water damaged ceiling tiles throughout the facility. Repair.

- Found 5 badly dented cans of food on shelves. I separated them and pointed them out to the manager.
- The counter and shelves in the deli have chipped paint or laminate missing. Observed duct tape being used on door to meat room as well as there being large holes on the door. On shelves in the meat room and walk in cooler off of meat room there is paper. Make sure all surfaces throughout the facility are easily cleanable. Some of the walls of the walk in coolers are rusting.
- There is dust accumulated in the fans of the walk in coolers for meat and dairy.
- The faucet at the 3-compartment sink leaks.
- Found no lid on waste can in women's restroom.
- Observed damaged floor tiles throughout the facility.
- There is black buildup around the walk in freezer.
- Observed holes and chipping paint throughout back room on walls and ceiling. Observed peeling wall paper in grocery store. There is a drip coming from the ceiling by the water bottles. There have already been tarps used to protect foods/drinks.

- Found spoiled food in facility. Upon inspection manager got rid of spoiled apples and peaches.
- Observed employee in the meat room not wearing effective hair restraints.
- Observed shelf under meat slicer chipped. The counter in the deli is damaged. Observed seal on cooler with milk in it broken. Observed duct tape being used on door to meat room. On shelves in the meat room there is paper. Make sure all surfaces throughout the facility are easily cleanable.
- Observed air conditioner in meat room leaking as well as the fan in the walk in cooler that holds meat. Make sure to not store any food or food contact surfaces underneath as this isn't potable water.
- Found no lid on waste can in women's restroom.
- Observed damaged floor tiles throughout the facility.
- Observed holes and chipping paint throughout back room on walls and ceiling. Observed peeling wall paper in grocery store.
- Lights in meat room missing caps.
- Observed nail polish remover on a shelf in the meat room. Discussed with manager that it should be away from food contact surfaces.

- There is laminate and paint coming off counters in the deli. The door to the meat room has holes and duct tape around the window.

- The lights in the meat room have the tubes, but are missing the black end caps.
- Found 3 cans of raid stored with cleaners in the back of the facility. Per manager they were old and they didn't use them. He voluntarily disposed of them.

- There was a fly strip in the deli area over the stove and fryer. Even though it isn't directly over there is still a chance of a fly falling in food.
- The counters and shelf in the deli area have cracks and coming off in places. The seals of some of the walk in coolers/freezer are damaged. There is a ruler taped on the meat saw.
- Food damaged/missing floor tiles throughout the facility.
- There is missing coving in different places throughout the facility.
- There are several places where the walls are damaged or missing paint. Also the door to the meat room has a hole in it. There are ceiling tiles that are stained or damaged.
- Several lights are missing shields.
- There are discount carts at the front of the store. One cart was intermingled with soap, medicine, light bulb, and raid. This cart had the food taken out and disposed of. Discussed possible contamination and they must be separated.
