- Observed open containers of flour on shelf under prep table in the kitchen. Cover food in storage to prevent contamination. Corrected.
- Observed employees not wearing effective hair restraints. Provide hats, hairnets, or other hair coverings.
- The metal shelves in the walk in cooler were rusting. Paint or replace them.
- The dry storage shelf under the potatoes had a buil up of debris. Clean and sanitize it.
- The microwaves, stove, and Southbend oven had a build up of debris. Wash, rinse, and sanitize these areas.
- The cooler, freezer, and cabinet door handles had a build up of debris. Clean and sanitize these areas.
- Observed mouse feces and signs of chewing/holes at the bottom of the east wall in the furnace room. Clean the floor. Repair the wall so it is impermeable to rodents. Take steps to eliminate rodents.
- Clean the floor in the walk in cooler to remove water. The walk in freezer floor had a build up of debris. Clean the floor.
- Replace the damaged ceiling tile in the banquet room.

- Found a gallon jug of milk on the counter in the prep area in the kitchen at 68°F. It must be kept at 41°F or below. It was discarded by the manager.
- Observed employee shutting off the faucet with his hands after washing them, recontaminating his hands. Employee was retrained on site on proper hand washing procedures and demonstrated proper hand washing to inspector.
- Observed employees not wearing effective hair restraints.
- The chlorine wiping pails were at 50 ppm. Maintain at 100-200 ppm. Corrected by changing out sanitizer water to be 100 ppm chlorine.
- The cooking equipment in the kitchen and microwave interior in dish room had a build up of debris. Clean and sanitize these items.
- The outsides of coolers and cabinets in kitchen had a build up of debris. Clean and sanitize them.
- The dining room carpet was torn in several places. Repair or replace it. Them kitchen floor is chipping paint in the kitchen. Repaint these areas.
- Found build-up of debris on floors. Clean the floors.

- Found cooked and cooled foods throughout the coolers and freezer that were only labeled with the date. These items were recorded on a cool down chart. Label these foods with both the date and time of onset of cool down.
- Found a pan of raw chicken stored next to a container of cottage cheese in the grill prep cooler. Corrected It was moved to the other side of the cooler with the rest of the raw meat.
- Employees were not wearing hair restraints.
- The shelves, cabinets, and counters in the kitchen had a build up of debris. Clean and sanitize them.
- The microwave interiors, stove top, and Southbend oven had a build up of grease and debris. Clean and sanitize them.
- The drain pipe under the 3-compartment sink was broken. Repair or replace it. It got knocked off with the mop bucket. Owner fixed the pipe during the inspection.
- Found build-up of debris on floors. Clean the floors, including the men's restroom floor.
- During today's inspection it was noted that your facility is in violation of the food handlers requirement (750.570). This requirement is to be corrected by your next routine inspection.
