- Observed two separate sandwiches listed with discard dates of 2/23 & 2/24 in the refrigerator. All potentially hazardous items must be discarded or used-by the 7-day date label. Manager discarded the items during inspection.
- Observed two tubs of meat stored on the floor in the basement freezer. All foods must be stored a minimum of 6
- Observed build up of ice inside of white cooler/freezer in service area. Food contact surfaces must be smooth, easily cleanable, and in good repair. Defrost cooler/freezer on a routine basis to permit proper cleaning.
- Observed a hole in the floor beneath the chips in the kitchen, damage to the wood floor in the dining area, and broken/damaged tiles in service area. Repair all floors to permit proper cleaning.
- Observed floor to wall coving along back wall beginning to peel away allowing food debris to deposit inside. Repair floor to wall coving to allow for proper cleaning.

- Observed the following items without proper date labeling or past 7 day expiration date: open sour cream, sauerkraut, and pepper jack cheese. All potentially hazardous foods open/removed from original container must be labeled with a 7 date label. Manager voluntarily disposed of all items during inspection. Provide TCHD with signed training documentation for seven day date labeling procedure in HACCP plan by 12/13/15.
- Observed plastic cutting boards with deep abrasion, discoloration, and melted corners. All food contact surfaces must be cleanable, smooth, and in good repair. Repair or replace cutting boards as necessary to allow for proper cleaning.
- Observed paper towels used to absorb water from upside down espresso utensils. Food contact surfaces must be non-absorbent and self-draining to prevent contamination of sanitized equipment. Employee removed paper towels from food contact surfaces.
- Observed a unused oven used for storage of cooking pans. This oven is considered a food contact surface and must be cleaned as regularly as necessary to protect against contamination.
- Observed a build up of debris on equipment underneath large refrigerator in kitchen. Clean as routinely as necessary to prevent contamination.
- Observed damaged floor tiles behind cash register. Repair to ensure proper cleaning.
- Observed a hole in the wall behind the dish machine. Repair to allow for proper cleaning.
- Observed a build up of dust and debris above the baking goods. The baking goods were beginning to collect dust also. Clean as routinely as necessary. An employee washed, rinsed, and sanitized all food containers that had dust.

- Observed employees prepping food without any hair restraints. Always wear a hair restraint (hat, hair net, etc) when prepping food.
- The seal in the reach in cooler/freezer in the kitchen prep area is broken and being held on with tape. A maintenance man was there at the time of inspection to install a new walk in cooler and repair the seal.
