- Observed the following areas of the floor in need of repair: damaged tiles in front of 3 compartment sink, around water heater, and entrance to the back storage room. Repair as routinely as necessary.
- Observed the following floor with a build up of debris: under shelving in back storage area, by/under soda lines, underneath soda machine in drive through, floor of the storage shed. Clean as routinely as necessary.
- To be in compliance, facility must create a log of all employee's names followed by the date of their FSSMC or Food Handler certifications. All new employees must include their hire date.

- Observed the following non-food contact surfaces with damage: handle of thermo unit and outside of deli slicer. Repair.
- Observed sanitizing cloths stored on counter. These must be stored in sanitizing solutions between uses. Corrected by employee on-site.
- Observed build-up of debris on pop machine in drive through. Clean routinely. Observed build up on white plastic drawers on table, clean routinely.
- Observed the following to have a build-up of debris: wheels of portable shelving, wheels of portable equipment, portable fans, behind TV monitor, and beneath soda machine CO2 machine. Clean routinely.
- Observed gap forming along bottom of back door and along outside of the outdoor storage shed. Establishments should have the outside of building protected form pest entrance. Facility has 10 days to repair or replace to deny pest entrance.
- Observed large opening in wall in storage room located in customer area. This provides pest harborage areas or entrance into food prep area. The facility has 10 days to repair.
- Observed the following areas with broken tiles: in front of door entering kitchen from dining, and around water heater drain. repair to allow for proper cleaning.
- Observed build-up of debris on floors throughout facility especially: under the 3 compartment sink, around grease trap, under shelving, floor behind compressed gas tanks, by electrical conduit under breaker, under tables, and under equipment. Clean routinely.
- Observed a hole around pipe in ceiling allowing entrance and missing coving above mop sink. Repair.
