  • Fast Corp #F631 Ice Cream Machine: 4.5 Clean the dusty freezer exterior in order to keep equipment clean. NOTE: Unable to test the health timer today. Please establish a knowledgeable contact person to ensure/test the safety of the vending machines.
  • Crane National #673/Hot Beverage: *4.5/4.6 Observed the liquid build-up on the coffee dispensing nozzles. Food contact surfaces shall be washed, rinsed, and sanitized and clean to the sight and touch.
  • 4.5 Clean the exterior of the powder containers and the interior of the machine in order to remove debris build-up. NOTE: Health timer was working properly.
  • National Vendor cold foods: R/3.5 Observed the ingredient labels lacking the name and place of business where the foods were made. Provide complete ingredient labels on all foods in this vending machine. NOTE: Unable to test the health timer today.
  • Please establish a knowledgeable contact person to ensure/test the safety of the vending machines.
  • KITCHEN:
  • 3.2 Label the bulk flour bin in order to prevent confusion or misuse.
  • R/4.4 Observed ice being used as a coolant in place of mechanical refrigeration on the salad bar. Repair this unit in order to keep equipment in good repair.

-Observed proper time-in-lieu of temperature procedures.
-2 accurate digital thermometers available for use.
-Observed proper consumer advisory posted for made-to-order eggs.
  • R/4.4 Salad bar is not functioning and operation is using ice as a means of cooling. Repair this area.
  • R/6.4/4.5/4.4 The following areas were soiled or damaged:
  • - leaky Imperial Fryer unit
  • - damaged wall strip on the corner by the convection oven
  • Clean or repair these areas. Equipment shall be maintained in a state of good repair.
  • R/4.4 Repair the damaged cabinet surfaces on the counter/cabinet unit where the Pepsi machine will be relocated. Contact MCHD
  • Plumbing division for any permits or approvals needed to relocate the beverage dispensing machine at 330-723-9523.
  • Note: Operation provided information for the kiosk in the dining room. See the FAX received by this office on May 16, 2011.
  • License transfer will be submitted for appropriate approvals.

  • Note: This inspection was conducted to assure the operation was in compliance to transfer the license. The same staff and PIC are running this FSO. The only change on the current FSO license will be the license holder name. This inspection will count as the first standard inspection for the 2011-2012 licensing year. Operation was previously approved for Time-in-Lieu of temperature procedures for several food items, since same procedures are being followed by the same staff, approval will follow the new license holder. See the file at Medina County Health Department for food items included in this approval.
  • Sanese Services will be the new license holder following the transfer. Reportedly, the current vending machines operated by the
  • FSO staff will be removed and a self service kiosk center will replace them. Provide this office with a plan for the new kiosk area including the make and model number of the equipment and the location of the equipment.
  • The following vending machines were added and inspected at this location:
  • Fast Corp/ #F631 Ice Cream Machine: No violations were observed on this machine.
  • Crane National Vendor Sure Vend/#673 Hot Beverage: No violation were observed on this machine
  • National Vendor/Shoppertron cold foods: 6.2 Repair the non-functioning light fixture in this unit
  • 4.1/9.0 Operation added 3 Sharp non-commercial microwaves. Remove non-commercial equipment. Provide this office with the make and model numbers of the new commercial microwaves.
  • Additional violations/comments: Operation replaced the Lincoln Impinger pizza oven with a newer Lincoln Impinger Pizza Oven.
  • *4.4/4.5/4.6 Observed the dish machine contained 0 ppm of chlorine sanitizing solution during the rinse cycle, and the third vat of the three vat sink contained 0-100 ppm of QAC sanitizer. Maintain chlorine sanitizer at 50-200ppm, and QAC sanitizer at 200 ppm in order to effectively sanitize. Utilize Bleach in the three vat sink to manually sanitize until repairs are complete.
  • 6.4 Observed unnecessary people in the kitchen. Only FSO staff is approved in the rear food preparation area, and behind the sneeze guard.
  • R/4.4 Salad bar is not functioning and operation is using ice as a means of cooling. Repair this area.
  • 6.4/4.5/4.4 The following areas were soiled or damaged:
  • R/- floor/wall junctures and floor surface in the dish area
  • - leaky Imperial Fryer unit
  • - door seals on the Beverage are unit
  • - damaged/lacking caulk at the wall/ three vat sink juncture
  • - damaged wall strip on the corner by the convection oven
  • Clean or repair these areas. Equipment shall be maintained in a state of good repair.
  • 6.4 Repair the wall surface by the hot holding cabinet and pizza reach-in unit. Provide the outlet cover to provide cleanable surface.
  • 4.4 Repair the damaged cabinet surfaces on the counter/cabinet unit where the Pepsi machine will be relocated. Contact MCHD
  • Plumbing division for any permits or approvals needed to relocate the beverage dispensing machine at 330-723-9523.

R/6.3/PROTECTION FROM CONTAMINTION: Observed three dented cans of Refried beans. Packages of foods shall be in good condition to protect the integrity of the contents and to prevent potential contaminants. These canned good was removed from normal stock.
  • 6.4/4.5/4.4 The following areas were soiled or damaged:
  • R/- floor/wall junctures in the dish area and floor surface under equipment as needed
  • R/- observed the open air case appears to contain a leak
  • R/- the floor, walls, base cove, and counter surface in the women's restroom were damaged or soiled
  • - door seals and bottom of the Beverage-Air unit was soiled
  • - canned food rack is soiled
  • Clean or repair these areas.
  • 3.2 Label the water squeeze bottle with the name of the food.
  • 4.4 Salad bar is not functioning and operation is using ice as a means of cooling. Repair this area.
