  • 3.4 Lunch meats were lacking date marking, sandwiches were dated 1/8/12, and a roaster of food in the walk-in cooler was lacking a date mark. Ready-to-eat foods shall be date marked when opened and consumed within 7 days.
  • *3.2 Observed the raw meat was stored next to pepperoni and above other ready-to-eat foods. Different types of raw animal foods shall be stored separate from other raw or ready-to-eat foods to prevent cross-contamination.
  • *7.1 Observed that disinfectant spray was stored next to the sandwich bags. Toxic chemicals shall be stored in a separate designated area from equipment.
  • *4.5/4.6 Observed the fryer baskets and tongs were soiled. Food contact surface shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • R/6.2 Provide a hand washing sign at the hand sink to inform employees when to wash hands.
  • 6.4/4.5 Clean the following areas:
  • - Floor and wall surface throughout the rear storage and equipment washing area
  • R/- Provide lacking base cove in the newly repaired wall surface in the rear storage area (See Compliance Agreement dated June
  • 23, 2011, this area was to be corrected by July 31, 2011).
  • - Free-standing fan is dusty
  • R/- Exterior of the oven, exterior of all equipment and shelves in the rear storage area to remove construction dust
  • 6.4 Repair the following uncleanable areas:
  • R/- Newly repaired wall surface, and floor surface in the rear storage area, to provide a cleanable surface
  • 6.4 True reach-in unit, and small chest freezer was not being used during this inspection. Remove unused unnecessary equipment from premise.
  • 4.1 Provide a thermometer in the bottled beer cooler with milk, current thermometer is in disrepair.
  • 4.5 Clean the slicer blade guard and surrounding area which is soiled.
  • 4.8 Store foam cups six inches off the floor to facilitate cleaning and to prevent contamination.
  • 6.2 Non-functioning light fixture above the three-vat sink. Repair this to provide 20 foot-candles.
  • 4.0 Repair the damaged wooden cutting board in the rear storage area. Equipment shall be maintained in a state of good repair.

  • *7.1 Toxic chemicals were stored with straws. Toxic chemicals shall be stored in a separate designated area from equipment.
  • 6.4 Provide pest control measures for the fruit flies or flying insects.
  • 6.2 Provide hand washing signs at the hand sink, to inform employees where to wash hands.
  • C/3.2 The ice scoop was stored in the ice machine. Store the ice scoop on a clean and dry surface.
  • 6.4/4.5 Clean the following areas:
  • - Floor surface throughout the rear storage and equipment washing area

  • - Exterior of the oven, three-vat sink, and shelves in the rear storage area to remove construction dust
  • 3.2 Observed bottled milk was stored in the ice bin. Food shall not be stored in a bin with consumable ice to prevent contamination.
  • 6.4 Repair the following uncleanable areas:
  • - Newly repaired wall surface in the rear storage area, to provide a cleanable surface
  • 6.1 Keep the rear door closed or provide a tight fitting screen door with a self closure to prevent pest entry.
  • Note: True reach-in unit was not being used during this inspection.

  • R/6.4 Repair the base cove by the freezer units in the rear room, also repair the floor/wall juncture by the chest freezer. Wall surface is lacking behind the oven and freezer units. Repair these areas.
  • Notes: Plumbing permit was obtained from this office on 6/22/11. During future inspections by this office, the Plumbing inspector must verify the plumbing on the one vat sink in the rear area is indirectly drained and can be utilized as a preparation sink for washing produce.
  • A compliance agreement was created today by this office and the PIC, correction date for the repairs listed above is July 31st,
  • 2011.
  • Menu was obtained today.
  • Three persons have completed Level One training.
  • Contacted Don Stewart, Deputy Health Commissioner, today and notified him the results of this inspection. All items listed on the
  • Board of Health Motion have been completed. Operation is now permitted to re-open.

  • R/6.2 Provide 20 foot-candles of lighting above the restroom hand sinks, currently 4-5 foot-candles above both restroom hand sinks.
  • R/6.4 Repair the base cove by the freezer units in the rear room, also clean or repair the mud at the floor/wall juncture by the chest freezer. Wall surface is lacking behind the oven and freezer units. Repair these areas.
  • 9.0 Observed a new kitchen area was constructed and was currently in use. This area is not approved for use, and is lacking any plumbing permits and approvals from any other applicable agencies. Submit plans and appropriate fee, to this office containing a layout, make and model numbers of equipment, surface finishes of floors, walls, and ceilings.
  • Note: New area contains existing equipment and several new pieces of equipment.
  • R/6.4 Ceiling surface in the newly constructed kitchen is textured and is not cleanable. Repair this area to provide a cleanable surface.
  • *4.5/4.6 Soiled ice maker shield. Wash, rinse, and sanitize the ice machine shield.

  • *4.4/4.5/4.6/4.2 Currently no three-vat sink is available for use in the new area. Existing three vat sink in the rear area was lacking soap, sanitizer, and test kit.
  • Reportedly, existing sink is used for big pieces of equipment and small utensils are washed in the hand sink in the new area.
  • Wash, rinse, and sanitize, equipment in the three vat sink. Provide sanitizer, soap and test kit.
  • R/*2.4/5.1/6.2 Observed no soap is available at the new hand sink. Provided this in order to effectively wash hands.
  • R/6.4 Ceiling surface in the newly constructed kitchen is textured and is not cleanable. Repair this area to provide a cleanable surface.
  • Note: New area contains existing equipment and several new pieces of equipment.
  • Discussed with PIC the food left over from the dart banquet, such as but not limited to roast beef and gravy, mashed potatoes, green beans ,corn and salad.
  • Reportedly the owner has been experiencing some recent health issues.
  • Additional violation :
  • *4.5/4.6 Wash, rinse, and sanitize the ice machine shield and the the three vat sinks contain a mold-like growth.

  • 3.4 Pepperoni was dated 1/19/11, salami was dated 1/19/11, and the ham was dated 1/12/11. Ready-to-eat foods shall be date marked when opened and consumed within 7 days.
  • *4.5/4.6 Observed a glass with mold-like growth contained a thermometer covered in a mold-like growth, also the pop gun holders were soiled. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • R/4.4 Repair the damaged ice machine shield, the ice machine also contains a part hanging into the ice bin. Equipment shall be maintained in a state of good repair.
  • 6.2 Provide 20 foot-candles of lighting above the restroom hand sinks, currently 12-13 foot-candles above both restroom hand sinks.
  • 6.2 Light fixture in the room with the existing three vat is non-functioning. Repair this.
  • 6.4 Repair the base cove by the freezer units in the rear room, also clean or repair the pooling water at the floor/wall juncture by the chest freezer.
  • 4.4 Resurface the wood equipment shelves in the room with the existing hand sink, to provide a cleanable surface.
  • *2.2 Observed employee cut lemons without gloves. Employees shall use gloves when handling ready-to-eat foods.
  • 9.0 Observed a new kitchen area was constructed and was currently in use. This area is not approved for use, and is lacking any plumbing permits and approvals from any other applicable agencies. Submit plans and appropriate fee, to this office containing a layout, make and model numbers of equipment, surface finishes of floors, walls, and ceilings within 30 days from the date on this report.
  • The following violations were observed in the new area:
  • *4.4/4.5/4.6/4.2 Currently no three-vat sink is available for use in the new area. Existing three vat sink in the rear area was lacking soap, sanitizer, and test kit.

  • Note: New area contains existing equipment and several new pieces of equipment.

  • 3.4- Lunchmeats and chicken wings were lacking date marking. Ready-to-eat foods shall be date marked when opened and consumed within 7 days.
  • *3.4 Observed raw hamburgers stored with the ready-to-eat foods in the refrigerator. Different types of raw animal food shall be stored separate from or not above ready-to-eat foods to prevent cross contamination.
  • *7.1 Observed one unlabeled chemical spray bottle hanging on the mop sink. Toxic chemicals shall be properly labeled with the container contents.
  • *3.0 Observed mozzarella cheese and ham contained a pink mold-like substance. Foods shall be safe, wholesome, and unadulterated. Discard these food items.
  • R/4.5 Observed the interior of the refrigerator contains a spill. Clean these areas.
  • R/6.4 Repair the damaged floor tiles by the refrigerator, to provide a cleanable surface.
  • 4.5/6.4 Clean the ceiling in the walk-in cooler, light fixtures in the walk-in cooler, and the exterior of the upright freezer which are soiled.
  • 6.4 Clean the floor under the white counter, and floor/wall junctures under hand sink.
  • 4.4 Repair the damaged ice machine shield. Equipment shall be maintained in a state of good repair.
  • 5.4 Provide pest control measures for the fruit/drain flies.
