  • On this day this inspector attempted to contact the above operation, the phone continuously rang. This inspector also attempted to contact the operation by phone on 2/8/12, and 2/14/12 with the same results. Melissa O'Connell and Lisa Vealey with the office also stopped by this operation on 2/14/12, and observed the lights were out in the operation, no gas prices were available on the sign at the street, and a sign was posted on the door stating the operation was closed for remodeling.

  • R/*7.1 Observed a variety of chemicals were stored above equipment on the rear storage shelves. Store toxic chemicals in a separate designated area from food and equipment to prevent contamination.
  • R/*3.4 Observed Egg, cheese, and ham muffin was holding at 130-134°F, also the American cheese was holding at 51-53°F.
  • Maintain foods at 41°F and below or 135°F and above to limit the growth of organisms.
  • R/*3.4 Observed roast beef was date marked 9/21/17. Foods shall be dated the day of opening or preparation and shall be used or discarded in seven calendar days.
  • R/4.4 Repair the damaged door seal on the reach-in unit.
  • R/6.4 Continue to provide pest control measures for flies throughout the operation.
  • R/6.4 Observed a reach-in cooler was in disrepair and was relocated to the shed, also additional equipment was observed in the shed. Continue to remove any unused unnecessary equipment.
  • R/4.4/6.4 Continue to make repairs on the ice machine or remove this unused unnecessary equipment from premises.
  • R/6.4 Cat was in the RFE. Animals are prohibited in the RFE.
  • Note: Reportedly pop dispensing machine is scheduled to be removed.
  • Discussed the air temperature of the paper lined bin, where sandwiches are stored with PIC.
  • Additional violation:
  • *3.0 Observed the tomato in the top of the reach-in unit contained a mold-like growth. Food shall be safe wholesome and unadulterated. Discard this food item.

  • R/*3.1 Observed a can of cream of mushroom were dented on the seam. Packages of food shall be in good condition to protect the integrity of the contents and to prevent potential contaminants.
  • R/*5.1/2.4/6.2 The hand sink was non-functioning and was blocked by broom, Dirt Devil, and tools. Make necessary repairs to provide hot and cold running water, and keep the hand sink accessible, to promote proper hand washing practices.
  • *7.1 Observed oven cleaner was stored with equipment, penetrating oil was stored with equipment and pan/cooking spray, and a crate of paints and maintenance items were stored above equipment in the shed. Store toxic chemicals in a separate designated area from food and equipment to prevent contamination.
  • *7.0 Observed a green spray bottle was unlabeled. Label toxic chemicals with the containers contents to avoid confusion and misuse.
  • *7.1 Observed Rid a bug ant and roach spray on the shelf above the three vat sink. Provide an approved pest control spray.
  • *3.4 Observed Egg, cheese, and sausage muffins were holding at 118°F, egg, cheese, and ham was holding at 124°F, American cheese was holding at 55°F, and pepperoni and cheese roll was holding at 112°F. Maintain foods at 41°Fand below or 135°F and above to limit the growth of organisms.
  • *3.4 Observed pepperoni, turkey, ham, and roast beef were lacking date marking and the egg patties were dated 8/9/11. Foods shall be dated the day of opening or preparation and shall be used or discarded in seven calendar days.
  • R/4.8 Store refrigeration racks six inches off the floor to facilitate cleaning and to prevent contamination.
  • R/4.4 Repair the damaged door seal on the reach-in unit.
  • 5.4 Observed boxes and garbage and outside the operation by the delivery door. Relocate garbage to the dumpster.
  • 6.4 Repair the lacking base cove under the hand sink, to provide a cleanable surface.
  • 6.4 Provide pest control measures for flies throughout the operation.
  • 6.4 Clean the floor/wall junctures under the three-vat sink, floor/wall junctures and floor surface under racks in both walk-in units, floor surface in the shed on the exterior of the operation were soiled.
  • 6.2 Repair the non-functioning light fixtures above the restroom hand sink, to provide lighting requirements as specified in OAC 3717.
  • 4.4 Make necessary repairs to the leaky ice machine, damaged counter strip above the reach-in unit. Equipment shall be maintained in a state of good repair.
  • 4.5 Clean pooling water in the light shield in the walk-in cooler, ice accumulation on the shelves, condenser, door frame and food products in the walk-in freezer. Clean or defrost these areas.
  • 6.1 Keep the front and rear doors closed to prevent pest entry.
  • 4.4 Provide a lid for the hot dog roller, to prevent consumer contamination.
  • 6.2 Relocate the hand washing aids from the three-vat sink. Make repairs to the designated hand sink.
  • 6.4 Observed a reach-in cooler was in disrepair and was relocated to the shed, also additional equipment was observed in the shed.
  • Remove any unused unnecessary equipment.
  • 6.4 Cat was in the RFE. Animals are prohibited in the RFE.
  • 4.8 Provide a lid for the stirrer dispenser to prevent consumer contamination.
  • 3.2 Store canned beverage in the ice in a manner to prevent contamination of the food/lip contact surface.
  • Notes: R/Observed the Captain Crunch Treasures cereal was dated 5/10/11. Check and rotate stock regularly, and remove expired food products from the sales floor.
  • Discussed container of pooling water and paper towels with PIC.
  • Reportedly pop dispensing machine is scheduled to be removed.

  • *3.0 Shredded cheese was showing signs of spoilage. Food shall be safe wholesome and unadulterated. Discard this food.
  • 4.8 Store refrigeration racks six inches off the floor to facilitate cleaning and to prevent contamination.
  • 4.4 Repair the damaged door seal on the reach-in unit.
  • 5.4 Dumpster and receptacle area contained excessive garbage. PIC contact dumpster service today for pick-up.
  • 6.4 Repair the lacking base cove by the retail shelves and the ATM, to provide a cleanable surface.
  • Notes: Observed the following food items beyond their sell-by/use by dates: (R)Stove Top stuffing dated 6/30/10, Fruit Cheerios bars were dated 11/27/10, and two boxes of Cinnamon graham crackers were dated 11/10/10, and 12/6/10. Check and rotate stock regularly, and remove expired food products from the sales floor.
  • Operation added Adcraft Panini machine model number 5G-811EB. Contact this office w/menu and equipment changes.
  • Hot Dog roller was not in use. Prior to using provide a lid for the hot dog roller.
  • Reach-in cooler was in disrepair and was relocated to the shed.
  • Discussed wicker baskets for the storage of raw eggs. Provide a tray or cleanable surface, to prevent contamination.

  • *5.1/2.4 Water to the hand sink was turned off because the sink was leaking. Make necessary repairs to provide hot and cold running water, and to promote proper hand washing practices.
  • *3.0 Tomatoes were showing signs of spoilage. Food shall be safe wholesome and unadulterated. These tomatoes were discarded today.
  • *3.4 The reach-in cooler contained and air temperature of 43°F, and the pepperoni and mozzarella cheese was holding at 42°F.
  • Maintain foods and refrigeration units at 41°F and below.
  • 4.5 Defrost the ice accumulation under the condenser and on the wall and door frame of the walk-in freezer.
  • R/6.1 Repair the air gap under the rear door to prevent pest entry.
  • R/6.2 Repair the non-functioning light fixture in the walk-in cooler to provide 10 foot-candles of lighting.
  • 6.4 Clean the floor under the walk-in freezer storage racks.
  • 4.4 Provide a lid for the hot dog roller, to prevent contamination. Reportedly, this lid is on order.
  • Notes: Observed the following food items beyond their sell-by/use by dates: Dill Pickle Chips 8/08, Stove Top stuffing dated
  • 6/30/10, and Cream of chicken soup dated 5/23/10. Check and rotate stock regularly, and remove expired food products from the sales floor.
  • Operation added Blue Bunny (Rio) Ice Cream cooler model number AHT S175 with ETL label. Contact this office w/menu and equipment changes.
  • Operation is using ZEP Food Service Heavy Duty Degreaser Product # RII 735, as an additive to the grease trap.
