- 6.1 Repair/eliminate the air gap under the rear door to prevent pest entry.
- 6.4 Relocate employee personal items from the food storage shelf to a separate designated area to prevent contamination.
- 4.5 Clean the door handle on the walk-in cooler, which is soiled/contains food accumulations.
- 3.2 Observed foods in the freezer were uncovered. Keep food covered when not in use to prevent contamination.
- R/3.2 Observed squeeze bottles throughout were unlabeled. Squeeze bottles of foods shall be labeled with the name of the food to avoid confusion and misuse.

- 6.4 Repair the damaged base cove throughout to provide a cleanable surface.
- 4.4 Repair the damaged caulk by the clear cup dispenser to provide a cleanable surface.
