  • On this date, this inspector attempted to contact Club Lafayette by phone the number has been disconnected. This inspector also attempted to contacted the operation on 2/14/12 with the same result. Melissa O'Connell and Lisa Vealey with this office drove by the operation on 2/14/12 and the operation was vacant with no tire marking in the snow. This operation is currently closed for business.

  • R/*5.0 This office received notification from the Ohio Environmental Protection Agency (OEPA) the permit to operate a public water system has expired. Anne Carney with the OEPA was contacted by phone during this inspection to verify if the permit was renewed at approximately 12 pm. Reportedly, she is unaware of the public water system permit renewal, however, she is unable to verify through the Ohio Attorney General's Office at this time. Per the PIC/Brett Cart a check for the public water system permit was mailed.

  • R/4.5/6.4 Clean the fan covers and ceiling in the walk-in cooler, to provide a clean surface.
  • R/6.4/5.1 Reportedly, a leak occurred under the bar 3-vat. Repair this area & clean the floor as needed in this area.
  • 4.4/6.4 Repair or remove from premise the non-functioning beer bottled cooler. Currently the operation is using tubs with hoses for bottled beverages. Also, this unit is rusty on the interior repair this.
  • R/6.4 Repair the ceiling & wall surface surrounding the new rear door.
  • 4.4/5.1 Finish the window surface in the rear door, & (OK) provide a weater strip on the bottom of the door to provide a cleanable surface and to prevent pest entry. Weather strip was clear in color.

  • Foods shall be dated the day of preparation & shall be used or discarded in seven calendar days.
  • *3.2 Raw bacon was stored above beverages in the walk-in cooler. Different types of raw animal foods shall be stored separate from or not above ready-to-eat foods to prevent cross-contamination.
  • 6.4 Provide pest control measures for drain flies.
  • 5.1 Repair the leaky hand sink plumbing, and the plumbing on the preparation sink.
  • 6.1/6.2/4.4/6.4 The following areas were damaged & uncleanable &/or soiled:

  • - light fixtures in the kitchen and walk-in cooler are nonfunctioning or soiled
  • - floor around & under the three-vat sink is soiled/damaged
  • - floor around the bar beer cooler is soiled
  • - beer cooler is rusty & non-functioning
  • - bare wood floor around the toilet in the men's restroom
  • - ceiling in the men's restroom is damaged
  • - the walls in the women's restroom are soiled/stained
  • - walk-in cooler condenser covers are dusty
  • - ceiling in the kitchen is soiled
  • - provide lacking light shields in the kitchen
  • - clean the floor/wall juncture throughout as needed
  • Clean or repair these areas.
  • 3.2 Solo cup was used as a scoop in the sugar. Provide a handled scoop.
  • Note: Stainless bins, hose for drainage, and ice is used to cool bottled beverages.
  • Provide water sample results to this office.
  • No food was prepared during this inspection. Kitchen currently does not have functioning refrigeration, and uses a bin with ice.
