KDA_3_50115A - --Canned green beans were placed into stainless steel pans with lids (at an appropriate product depth) at about 0930 as per employee. Lids were placed on the containers and they were placed in the walk in cooler. Two containers were stacked. This was the cooling method, no stirring etc. Had employee removed lids, stir up product, and monitor temperatures. At 1015 when first observed, the product ranged from 60-66 degrees.
KDA_4_20211A2 - --Two plastic pitchers hanging on a rack were cracked. This prevents proper cleaning and pieces of plastic can end up in food (discarded).
KDA_5_20515B - --On the mop sink, the faucet has a built in atmospheric vacuum breaker (AVB). Attached to the faucet is a brass AVB which failed and leaks when the water is turned on. Downstream of this ACB is a chemical dispensing unit with a built in backflow prevention device. The brass device can just be removed.
KDA_6_501112 - --Dead insects on the floor by the overhead receiving door.
