Well, this used to be Milan out of these 4 city options, but the Italians tried to change my mind and opinions for Milan. I now choose Madrid.
Toronto is kind of disappointing for the premier city of Canada, so it is distantly in last place. Montreal and Vancouver are more impressive and scenic cities compared to Toronto.
Madrid is the most impressive, sophisticated, and intriguing city out of these 4 options and wins in most of those categories. The obvious exceptions are beaches, diversity, and food scene for Sydney, and Toronto.
Despite what some Italians believe, Milan is a city to appreciate and respect, and has some qualities to enjoy for the city of Milan.
Toronto can never presume to be as scenic as Vancouver and although I agree that Montreal is also more scenic than Toronto, Toronto is far and away a more urban and cosmopolitan city than Vancouver. Additionally, it is the only real large city we have in Canada - it's metro area is almost twice that of Montreal.
You are entitled to your opinion naturally, but as with any vacation - it really depends on what you are interested in which will shape your view of which one is the best to visit. Toronto has a huge theatre scene and is the sight of intense urban renewal and densification. It is also a decent foodie city for authentic and diverse cuisine and also happens to have a pretty decent nightlife and wonderful nabe's - it's just a city you have to go off the beaten path to appreciate fully and like Montreal - has an impressive number of summer festivals that would rival most any city in the world for those who do some research into the places they visit.

Madrid seems the most interesting in terms of the actual city. I've only been to Sydney and Milan, Milan isn't as interesting as Rome IMO, but has it's points. I've been to Sydney tons of times, it sure is a beautiful city, and offers the best in terms of nature.
