The onus is on your new supervisor to make the transition happen. HR has been notified of the change in job classification and new title with an effective date. She has to make it happen.

I'm hip to your struggle tho, as I gave HR 5 weeks notice, and this is the last week....

That's a choice you're making. When I leave one job for a new one, the new job is my priority. I'll do what I can when I can for the old one for two weeks, but I'm gone. If the new job wants me for something, they're my first priority, because I'm more interested in the future than the past.
This is an internal transfer. It's not like quitting one job and starting another. Most places I've seen the losing boss, the gaining boss, HR, and occasionally higher ups all need to be on the same page to make it happen. Occasionally I've seen the losing boss doesn't want to let go and so slow rolls the paperwork and the transition.
