In my experience, you have a better chance if you have a plausible reason for moving there (employers are nervous about investing in an employee who may decide they hate the area or want to go home in a year or so). Do you have family ties there? Spouse get a job there? Try to explain in your cover letter why you want to move.
Depends on the job. If it is a professional job, it is not uncommon at all to interview out-of-state candidates.
I'm not sure what you mean by professional, but I don't think that makes any difference. If the company needs the workers bad enough, they will take them from where ever they can. I've seen plenty of people from the south move here for $15/hr blue collar jobs.

When we were applying out of state my cover letter said my husband was considering a job offer in the area and his cover letter said my wife is considering a job offer in the area. We would consider any offer
I found a job first and lived here 3 months before he was able to move.

Those workers were likely worth $10/hr and didn't have much expense in moving. Moving is cheap when you don't have anything to move in the first place!
