As per the website I posted, you don't need smartphone, computer with access to the Internet is just fine. If you find yourself you can even print the Smart Card.
Not sure if you can access your Mom's medical info, though.
Not sure what the blue card/paper means - she got her first two at the community center and the booster at Kroger but they just gave her a piece of paper
Most people get just piece of paper/card. Like the one on the link I posted. She should get some confirmation for her booster shot, though. Did you call Kroger?
