DC takes the top spot. For comparison, Hawaii, which has always been known to be VERY expensive to live, comes in under DC at $85.32!

From most expensive to least expensive (Entire state is taken into account, NOT metro areas, except DC of course):

DC: $84.60
MD: $89.85
VA: $96.90

Prices in DC, even on basic things, are going to be higher since the average income is higher. Not to mention the tourists who are out of town and have no choice but to shop there (Can personally vouch for that after having to pay through the roof for basic necessities on vacation in NC)

It would be interesting if they did it by county/city though. Maryland is a very diverse state. Places like Bethesda and Potomac are A LOT more expensive to live in than, say, Hagerstown or any of the Eastern Shore. In Howard County, the median income is $46,594 vs Somerset county on the eastern shore at only $16,919 per person.
