I currently have this pending on UIA, my reason for leaving my job was "Quit - Personal Reasons", I worked at a primary care clinic, left due to studying for graduate school + currently live at home with my parents, who are considered high risk, so didn't want to take any chances of bringing COVID-19 back. Do I just wait to hear back? It does allow me to add "additional docs".
It seems like you're basing this on old unemployment qualifications, why wouldn't I qualify if I worked at a doctor's office and left due to COVID-19 concerns? There were other choices mentioning the impact of COVID-19, like quitting due to being at high risk yourself or caring for someone that may have it. It seems like living with individuals that are at high risk may fall into this category, so if I do need to appeal and it does get approved, I'll update.
from my understanding, you had to put covid-19 as the reason for not working in order for the CARES act to apply for unemployment benefits. If you just put that you quit your job, then you don't qualify.

It seems to me that you failed to read section 2102 of the CARES act for this individuals that will qualify for PUA

Quiting a job due to COVID-19 concerns would be eligible during this time, there wasn't an option to choose "quit due COVID-19 concerns - living with high risk individuals", so I obviously didn't want to lie and put "personal reasons", if they require more information, I'll add what I stated here.

The entire country could quit their jobs based on that criteria. We're all concerned about getting it or passing it on to our high-risk loved ones. It has to be more specific than that.

(Not an expert, just giving opinion)
Entire country? A good amount are teleworking. My perspective on this was I worked in a high risk environment, a doctor's office, I left before things were getting too out of hand (end of February) and it was not taken as serious as it is currently with all the restrictions in place. Yes, I understand other places like grocery stores are in the same predicament now, but at the time, everything was still "normal". If that reason isn't sufficient, that's fine; doesn't really hurt to apply.

Link to NJ's chart which gives good explanation. I know you're a different state but it gives you a nice breakdown and outline. You wouldn't be entitled under CARES for UI.
When you quit your employment, how and what did you tell your former employer?
