Hello , I've searched this forum for advice but I'm not sure what is the best way to approach the current situation right now. Any help would be greatly appreciated.
I've started a new job with a company for roughly 2 months with them and didn't pass the probation period or wasnt the right fit. Thus, i got terminated with them and am applying for UI right now. I should've browsed through the forum before I started applying for UI but I didn't. On my application, I put that I was terminated due to poor performance in which this is true, I should've just left blank but didn't. EDD has now sent me a letter for a scheduled phone interview with a questionnaire about the reasons I was terminated against the agency Talx. I read that it was better if I didn't answer the call and I believe this was probably the right approach, so I did that. Now EDD has sent me a letter Request for Eligibility Information DE4365 , which is pretty much the same, they want me to answers the questions , but I can write the answers and mail it back to them. Would it be best to ignore this letter and try to appeal or should I write and draft the answers on this letter and mail it back to them?
Below are the questions and my answers if I were to answer them:
What were your job duties?
How much experience do you have in this field of work? [5 years]
What standards were you expected to meet?
Why were you not meeting the expectations? [Lack of training and help]
How much on the job training did you receive when you started? - [I had minimal training total of probably 6-7 hours within 2 months]
Was there opportunity for further training? [No, I asked for more training and they said no]
Did you have any prior warnings? [ I was warned 1 week to improve before I would be terminated, I improved but apparently not enough ]
How do you feel you performed? [I tried to perform to the best of my abilities to do this job but its just impossible without proper training]
Theres probably a better way to answer these questions , but this is just some insight of what happened, any suggestions or help would be greatly appreciated. Thank you

Your best chance if denied is to APPEAL. Don't do the questions. It's NOT your job. Your employer can get you denied all by themselves without your help. If they don't want to do their job, then lucky you. You do NOT want to be fighting your own words.
