A lot of this depends on your travel style, though. If I'm spending a whole week in a city as a base, I usually take the train somewhere a little smaller too, as a daytrip.

As per above... I utilize the 'region' when in a city (rarely for me).
Exception is Amsterdam and DC where I spend a LOT of time biking their excellent venues.
SF I walk a LOT! Taking the free library tours (a few per day).
While in SA, I rent a $15 car and spend a day west (Fredericksburg, Bandera, Comfort...) and a day north (Wimberley, Canyon Lake, New Braunfels...). When in Europe I take a train, when in Asia I take a <$40 flight. Such as go to Penang, Bali, or Bangkok while in Singapore.
of course I get a LOT of my adventure ideas (and things to do in a city) from getting info from locals, and NOT staying a a hotel (ick) YMMV! I usually spend my time doing stuff that is totally off the radar of a travel guide / hotel (ick) / tourist itinerary.

I have been living in a small city that doesn't have passenger train service for too long, lol.
For a day trip, it's ideal to limit the commuting time to 2 hours.
Paris and Istanbul are less safe than desired. Berlin actually sounds pretty cool, few of my acquaintances have been there yet.
I think Rome, London and Paris and their suburb are good for a week/6-7 days each. Berlin as well if you include the region, Potsdam in particular. Florence in my opinion is good for 4-5 days; it is just so small though really interesting...
Several cities in the US are well worth a week or more, including in my experience New York, New Orleans, Chicago, Philadelphia, Los Angeles, San Diego, San Francisco, Boston, Atlanta, and Washington DC. So are Canadian cities such as Toronto and Montreal.
In Europe, would definitely say Paris, London, Florence, Madrid, Vienna, Rome, and Amsterdam from experience. Copenhagen, Berlin, Munich, and Barcelona would also likely seem worth a week given what I’ve been able to figure.
Paris is unsafe? Since when? I've spent a lot of time there, never felt unsafe.
