I would recommend against renting a car if you visit Boston. They're more trouble than they're worth if you stay in the central parts of town. Good public transport from the nearby international airport, too.

In non of my foreign travel have I ever rented a car, so Belize, Peru, Cambodia, etc... all no car.

For cities, SF, Chicago, NYC, DC... all no car.

Definitely do not get a car for Boston.

In just about any European trip, particularly to the big cities, not only do you not need a car, but a car is actually a hindrance. You do not WANT a rental car.
In some countries you simply can't rent a car, or it is not advisable. India for instance (because most westerners will freak out at the method of driving) - you rent a driver if you need to get around, not a car.

In the US, either NYC, Boston, Philadelphia, Baltimore, DC, Chicago, New Orleans, or San Francisco are pretty easy to get around without a car.

If you are willing to travel abroad then I would highly recommend going to Europe or Asia since their public transport network is very good. Even Latin America is doable since many of those countries have a large public bus network that practically goes everywhere not to mention that taxi services tend to be relatively cheap.

Switzerland has the Swiss Pass that covers Trains, busses, even some funiculars and cable cars. Very easy to get out to nature this way. The tourist office will have guides to transit accessible hiking.

Venezuela was very easy without a car as they have "por puestos" which are shared taxis. But it's not recommended with the current political climate. Columbia appears to have changed for the better in the meantime. Lots of poorer countries will have jitneys available.

Venezuela also has insanely expensive airfares right now because the government is impounding airfares paid by Venezuelan customers and the remaining airlines flying into the country are passing that cost along to everyone else. I did a price check for some random dates in February and Boston-Caracas was $1000+ while Boston-Bogota, Columbia was $431.
