Old movie with Clint Eastwood by the name of "The Beguiled". Those sweet, young, vindictive southern belles were really disturbing.

I have seen several that weren't mentioned in the first eight pages of this thread:

Martyrs
Cutting Moments (part of Douglas Buck's "Family Portraits")
Vistor Q

However, the #1 worst ever has to go to August Underground's Mordum. While the original August Underground as well as August Underground's Penance also fit squarely in the "WTF did I just watch?" category, Mordum is in a class all its own. This truly is the sickest of the sick with completely realistic scenes of murder, torture, necrophilia and gore. This is not a film for anyone but the truly hardcore set. Caveat emptor!

I would also agree with the aforementioned Salò or The 120 Days of Sodom and Idi i Smotri (Come and See). Salò in particular made me want to take a Silkwood shower after watching it.

I saw the 120 days of Sodom, pretty sick stuff. I also saw some movie called "men Behind the Sun" that had a real clip of a mouse in a cage being eaten alive by 100s of rats.

and of course there's always that one weird snuff film with that japanese dude in samurai gear slowly cutting a chick into pieces. I think that's the film that Charlie Sheen watched, where he was so coked up, he thought it was real and called the FBI LOL.

The Flower of Flesh and Blood. It is grotesque, but not nearly as realistic as the entire August Underground series.

It's not a movie, but the HBO documentary 'White Light/Black Rain" really got to me. It truly made me cry.

I recommend watching the documentary.
