It's Halloween, let's think on suspenseful movies...that may or may not have monsters... and creepiest endings...ever! Must have both, OK?
I find creepy, horrifying, and other "non Hollywood" endings awesome, that's just my personality. Example: "Sixth Sense" was 1) suspenseful 2) supernatural 3) creepy ending indeed (that was also a shocker/revelation). Life isn't about happy endings.
One of my favorites: original The Hitcher (1986), remake in 2007 was dreadful. A human monster played with utter conviction by Rutger Hauer.

The Hitcher breaks out of prison transport/custody, stealing a SPAS-12 combat shotgun and killing his guards. As this occurs, the Kid (C. Thomas Howell) comes up behind the police transport in a stolen police cruiser. The Hitcher leaps from the back of the transport, onto (and into) the Kid's cruiser.
A gunfight of sort ensues, one sided, as the Hitcher toys with the kid in the now-stopped cruiser...after he's been thrown from the vehicle (back through the windshield). The Kid manages to start his cruiser, eventually, and runs down the Hitcher who only grins and invites him on to the attack.
After checking the body, prodding the Hitcher w/SPAS-12, the Kid slowly turns and takes a few steps....as the Hitcher, seemingly immortal, stands up behind and throws his chains at the Kid's feet with a bloody grin.
Not yet turning, the Kid pumps the shotgun's action. In a smooth motion, he pivots to blast the Hitcher. Then, once more. Finally, one last as the Hitcher's body flies off the road into the sand.
Fade to the day's dying light, slowly, with some of the best atmospheric music/soundtrack in the history of film: the kid walking, then leaning on the cruiser's smashed door, sound of wood match as he calmly lights a cigarette, melancholic music rising. All is finally quiet as the end theme and credits roll, holding that scene, and fading gradually to black.

I thought The Sixth Sense had a very happy ending. A little bittersweet? Yeah. But poignant and hopeful.

I don't think I've seen that since 1986, but yeah, it was good. The first movie I remember seeing as a kid where the hero utterly failed to save the girl.
I re-watched SILENCE OF THE LAMBS the other day. Still holds up. Great movie from beginning to end. Better than any thriller I've seen in a long, long time.

My top film of all time. Seen it many times. The suspense was the motive ( or lack of motive) of John Ryder-Who may not have even been real. The scene in the diner when Jim asks him why hes doing this, he says "your a smart kid, you'll figure it out" and puts pennies over the kids eyes was chilling. I liked the french fry scene also. The remake was awful, with the antagonist reduced to a one dimensional smirking thug.
A few others that are good-
Duel- Another stalked in the desert story, though it was a crazy truck driver. The film Joy Ride was similar and very good. The sequels not do much.
Black Christmas-Margot Kidder and friends get harrassed by mysterious and creepy stalker.
Hit and Run-A hit and run victim returns to get revenge. One of the creepiest villians ive seen. Very unique film style. Was from 2006 or 2007.
Halloween 1978- Needs no introduction. Imagine that creepy white mask slowly emerging from the dark. I did as a kid and was afraid to go out on the backyard of my apt complex. Seriously!
also stars Kiernan Shipka, who I've heard was in Mad Men.
