I'll go with The Exorcist. The scariest scene for me is when Regan's Mom walks in and Regan is being bounced up and down on the bed and she puts her head back with that gutteral growl while her throat swells up and her eyes roll back in her head.
also one of my all time favorites was the Zuni fetish doll from the 70's with Karen Black as "Amelia" you tube has the whole episode...love the little dolly that comes to life.

Although this movie was not technically a "horror" film, per se, but rather a Sci Fi flick, this scene posted below ranks in the Top 5 of "Most Shocking Scenes in Movie History," I think.
(A bit of trivia, no charge, from the DrummerBoy: Director Ridley Scott deliberately did not tell the cast members sitting at the table in this scene what was gonna happen, so he could catch their honestly-scared reactions!)
I will say the scene in Stir of Echoes with Kevin Bacon when he sits on the couch, leans forward to pick up a remote for the TV, sits back and there is a ghostly girl now sitting beside him.
