Does having a lapse in employment history due to a medical disability but than returning to the employer constitute a gap in employment (even though I was compensated by a supplemental long term disability plan)?

Medical issues are usually an exception to many things. Talk with your mortgage person and he/she'll be able to look at the details and determine if there's enough history.

Thanks, I talked to (3) mortgage originators and they said it should be fine. I am trying to see if underwriting might have an issue with it to prevent unnecessary expenses.
